I hereby apply for the grant of a scholarship. Necessary particulars are mentioned below:-

DECLARATION

signed) hereby declare that the particulars mentioned above are correct and true and that the amount of
scholarship. If awarded by the Trustees in their discretion, shall be utilised fully for pursuing my studies as
stated above and that I shall intimate to the Trustee in writing (with photocopies of certificates) the result of
each annual examination relating to the study course/degree being pursued. The Trustees may in their
discretion make reference directly to the University/College and send the amount of scholarship, if
awarded to the university/college.

5. Request for the renewal of scholarship whether annual or six monthly should be
sent in writing 30 days before expiry of scholarship with documentary evidence of
upto date progress.
Documents to be produce.
1) Attested (a) PHOTOGRAPH, (b) C.N.I.C (c) ENROLLMENT CARD (d) S.S.C (e) H.S.C/D.A.E copies.
b) FEE’S VOUCHER in respect of annual educational expenses incurred, duly verified by the
Principal/Chairmen of concerned department.

7) Original Certificate regarding the EXPECTED DATE FOR FINAL/ANNUAL year examination.
8) Certificate regarding the FINAL YEAR PROJECT (if any assigned from the chairman of the
concerned department, stating its TITLE, COST PER HEAD and the DATE OF ITS SUBMISSION.
9) An affidavit duly notarized and counter-signed by the DIRECTOR OF FINANCE (in case of
College Medical Student by the PRINCIPAL OF COLLEGE) stating that you have neither
received nor applied for any Scholarship / Qarz-e-Hasna or other financial aid during the current
year from any other source(s).
and further undertake that you will not apply for the same during the current year if Quaid-e-
Azam Aligarh Scholarship is sanctioned to you.
In case you have already applied for financial assistance from other source(s). Your case may
be reconsidered only if your application for the same is rejected and documentary evidence
regarding the same from the competent authority is duly produced to the Trust.

Instruction for filling Application Form for Quaid-e-Azam Aligarh Trust
2) The Candidate Must have neither received of nor applied for any other
Scholarship or Qarz-e-Hasna from any other Source(s).
3) He / She must have obtained first class marks in S.S.C and H.S.C. (D.A.E.)
and 60%/3.00 CGPA & above marks in each professional M.B.B.S. / B.D.S /
B.E.M.S / D.V.M. examination. While 70%/3.40 CGPA and above marks in
B.E. and other University post graduate Course (e.g., M.A / M.Sc / M.Com /
M.B.A. / L.L.M / M.P.A, Pharm-D) examination.
4) No scholarship can be granted unless the student has appeared in either the
first professional (Part I & II) examination (in case of Medical Student), and
the first University annual examination (in case of other student) and marks
sheet of said examination are enclosed with the application form.
5) Where results of any examination are awaited, the same should be specially
stated in the application form.
6) If you wish to avail of scholarship during the next year also provided you have
not appeared in your final year examination, request for renewal of
scholarship should be sent in writing before the expiry of period of
scholarship with the supporting documents.
