1. Who is responsible for each step?

• Officer on duty
Each step of the application will be handled by the officer on duty. Throughout the
process they will provide the clear guidelines to the applicant so that the applicant did
not get confused and the application process will run smoothly.
• Applicant
The applicant is responsible in fill up the application form correctly and provide the
necessary documents for the application process. It is because incomplete form will
affect the application process.

2. How is the flow of information activities?

To apply, the applicant needs to present to the Pejabat Kebajikan Masyarakat,
Pejabat Residen, Pejabat Daerah or Bahagian Pembangunan Awal Kanak – Kanak
nearby to obtain the application form. The applicant needs to fill in the form correctly
before the submission of the form. Upon the submission, applicant need to provide
the required documents such as photocopy of Kad Pengenalan Ibu Bapa (front and
back), Kad Pengenalan Penjaga (front and back), Sijil Perkahwinan, Kad Kesihatan
Ibu, Account Statement and Nota Discaj from hospital or Sijil Kelahiran for the
application process.

• BPAKK terima permohonan


Application needs to submit the form together with the required documents to the
office as stated above. Then BPAKK also known as Bahagian Pembangunan Awal
Kanak - Kanak will received the form with the required document to be processed.

• Semakan BPAKK
After that, BPAKK will run the checking process of the application form whether the
form is complete or not. This checking process by BPAKK usually will take 7 working
days.

• Permohonan tidak lengkap


If the form not complete and some of the required document missing, the form will be
returned to the applicant and applicant need to review back what is missing in the
form and submit the form back.

• Permohonan dilengkapkan dengan Nota Discaj/Sijil Lahir Bayi


This is the crucial part; the applicant must have the Nota Discaj or Sijil Lahir Bayi in
order to complete the application because if the applicant failed to present the
documents the application will not be processed.

• Proses pembayaran
Next, the payment process usually takes about 5 working days, and the funds will be
credited to the applicant bank account.

After the payment process complete, applicant will receive the email to inform the
funds already being credited to their bank account.
3. Does the workflow process facilitate work effectiveness?

In my point of view, this workflow is effective because it very organized and clearly
stated what the applicant should do in each step of the application process. Not only that, it
make the officer on duty work become easier because this workflow is very direct and not
complicated. This also reduce the error and chaos in the systems. Besides, the customer
service that been offered by the officer on duty is friendly and it will ensure the application
process run smoothly.

4. Are there any weaknesses of the work process flow?

In my opinion, this workflow does not have any weaknesses because it very direct.
Applicant just need to follow the instruction and submit everything that necessary for the
application. Besides that, if the applicant does not want to do the application manually, they
can do it online. They need to fill in form online and for the required documents, the applicant
needs to scan the documents and upload the document in the link that already provided.
