Instruction

DISCHARGING CLIENT

MATERIALS/EQUIPMENT:

1. Discharge Notice and Clearance


2. Discharge Notice Slip
3. Billing Checklist
4. Billing Logbook
5. Discharge Instructions (Alagang Pinoy Tagubilin / Baby’s Card)
6. Prescriptions
7. Wheelchair/stretcher (optional)
8. Decontaminating Solution
9. Rugs
10. Computer (Hospital Information System)

PRECAUTIONARY MEASURE/S:

1. Perform hand hygiene.


2. Wear appropriate personal protective equipment (PPE).

STEP-BY-STEP PROCESS RESPONSIBLE PERSON/S


1. Verify physician’s order. Nurse
2. Check for the completeness of the chart and PhilHealth requirements or Nurse
Point of Service and CF4 duly accomplished by the physician

3. Once with completed requirements ( Philhealth Clients ) accomplish the


billing checklist.
4. Tag as “May Go Home”. Nurse, Nursing Attendant
5. Forward the chart with triplicate copies of Discharge Notice and Nurse, Nursing Attendant
Clearance to billing section together with the billing logbook.

Note: If the client received blood transfusion, have the relatives settle the
used blood products by lending the blood clearance form and instruct
them to coordinate with the blood bank.

For Non-Philhealth clients, chart is not required.

6. Inform the relatives of their transaction number assigned by the billing Nurse, Nursing Attendant,
section. Administrative Assistant

7. Provide the client with instructions regarding home medications and Nurse, Nursing Attendant
follow-up appointments.

For all inborn babies, all Baby’s Card should be received by client’s relative
signed on the admitting logbook.

8. Issue Discharge Notice Slip upon settlement of account. Make sure the
client has accomplished the Client Satisfaction Survey Form. Discharge
client’s account in the Hospital Information system.

9. Accompany the client to the exit gates using wheelchair/Stretcher where Nurse, Nursing Attendant,
the security guard will remove the clients ID bracelet upon receiving Transporter, Midwife
Discharge Notice Slip.
10. Decontaminate/disinfect used materials (bed /bed side table) Nurse, Nursing attendant
immediately after vacancy.

Note: Patient record is forwarded to admitting section once patient has


been discharged.

*For death cases, patient’s record is forwarded to Hospital Information and


Records Management (8am to 5pm) or to Admitting Section after office
hours.
