AFFIDAVIT OF COHABITATION

We, TREVOR SCRIVENER and ANALYN PADILLA, British and Filipino citizen,
respectively, both of legal age and residents of Brgy. Awasian, Tandag City, Surigao
Del Sur, state under oath, THAT:

1. We are husband and wife, our civil wedding having occurred on August 12,
2014 before the sala of Judge Rufo Naragas of Regional Trial Court, Branch 40,
Tandag City, Surigao del Sur;

2. Before marriage, we have been living as common law partners since 2001 at
Brgy. Awasian, Tandag City, Surigao del Sur;

3. On that union, we are gifted with two (2) children, all female, a ten (10)-year
old and an eight (8)-year old;

4. I, TREVOR SCRIVENER, intend to stay here in the Philippines for good, and
have the desire to be naturalized as Filipino citizen;

5. We are executing this affidavit to attest the truthfulness of the foregoing facts
and for whatever legal purpose this may serve best.
