FAMILY SURVIVAL PLAN

1. What are the possible emergencies you and your family might face in your area of the country? List them down.
- Typhoons/Floods, House Fires, and Earthquakes

2. How will you and your family evacuate or escape your home if you need to?
- In case of an emergency my family will find all members and we can evacuate outside the house safely.

3. Where will you meet your family members if all of you are not home at the time of evacuation?
- Right outside of your home in case of a sudden emergency, like a fire. Outside of your neighborhood in case you
can't return home or are asked to leave your neighborhood. Everyone must know the address and phone number
of the meeting locations.

4. What route will you and your family take out of your neighborhood and town if you need to leave?
- A safe and wide route to pass through.

5. Do you have another route if needed?


- Waiting for instructions of the government they will provide the route where we can take.

6. What types of supplies will you and your family need to “shelter in place”? Do you have enough of these
items? (“Shelter in place” is the process of staying where you are and taking shelter, rather than trying to
evacuate.)
- Water (one gallon per person per day for several days, for drinking and sanitation), Food (at least a several-day
supply of non-perishable food), Battery-powered or hand crank radio and a Weather Radio with tone alert, Flash-
light, First aid kit, Extra batteries, Whistle (to signal for help)

7. What are your neighborhood or community warning signals (such as horns or sirens when a typhoon has
been seen in the area)? Do you and other family members know what they sound like and what they
mean?
- A speaker that will alert and inform us in case of an emergency.

8. What resources, organizations, and emergency services are in your community that can help in an
emergency? What is your backup plan if help is unable to reach you and your family?
- In a community its the LGU that will help individuals and families recover by ensuring that these services are
available and by seeking additional resources if the community needs them. Also, when an emergency occurs,
the local government uses all available media to publicize the types of assistance available and how to access
them.

wherever you are going?
- Usually we take our pets with us, we have leashes and a cage to put them on, although we can leave them to
our neighbors sometimes.

11. Do you have supplies prepared that you can take with you? (Often called a “go-bag”)
- NO

What supplies will you take with you? Enumerate specific essential items that is often found in a go-bag.
- a first aid kit, flashlight, extra batteries, matches in a waterproof container and a whistle. It also may be helpful to
have strong tape, plastic sheeting and a few tools.
