AFFIDAVIT OF CLOSURE

I, GEORGE P. AGUINALDO, of legal age, Filipino citizen, and a resident of Brgy. 48-B,
Cabungaan, Laoag City, Ilocos Norte, Philippines, after having been duly sworn to in
accordance with law hereby depose and states that;

1. I am the owner of a business registered under the tradename XANDER AUTO

SHOP located in Brgy. 48-B, Cabungaan, Laoag City, Ilocos Norte, Philippines;

2. The said business is registered with the Bureau of Internal Revenue (BIR) on
January 29, 2021 with Certificate of Registration Number 4OCN001495809;

3. That I am applying for the termination of the said business for the reason that the
same has not been earning any income;

4. That the business has no outstanding liability and obligation; and

5. That I am executing this affidavit to attest to the veracity of my statements and

for whatever intents and purposes it may lawfully serve.
