AFFIDAVIT OF CONTRIBUTION

We, the personnel of __________________________, all of legal age and with

office address at _______________________________________ after having been
duly sworn in accordance with law, do hereby depose and state.

The we, a member of the PNP, currently assigned in Aviation Security Group;

That we voluntarily willing to give/ contribute to any fellow Police Officers

assigned with Aviation Security Group through the AVSEGROUP “Pagtutulungan”
Project, if there is an unfortunate situations occurred that shall be limited to loss of life,
severe medical and injuries and said project is for organic personnel only of
AVSEGROUP;

We agreed for a contribution of Php50.00 for PCOs, PNCOs and NUPs will be
collected to support the funding to the following
 In case of death of AVSEGROUP personnel (Uniformed, Non-Uniformed)

That there shall be financial assistance to any active member of
AVSEGROUP, whether uniformed, non-uniformed personnel who died regardless of the
cause of death. The amount of Php50,000.00 will be provided to the family as financial
assistance by this Group;

 In case of serious illness of AVSEGROUP personnel (Uniformed and Non-

Uniformed Personnel)
That there shall be financial assistance to any active member of AVSEGROUP,
whether uniformed or non-uniformed personnel, who had serious illness such as:

- Cancer patients undergoing chemotherapy;

- Chronic kidney disease undergoing dialysis;
- Chronic Heath Failure;
- Stroke;
- Pneumonia;
- Tuberculosis;
- Cirrhosis; and
- Due for hearth operations or any related diseases that is due for
- emergency surgery
The amount of Php25,000.00 will be provided as financial assistance

 In case of serious injuries of AVSEGROUP personnel (Uniformed and Non-

Uniformed Personnel)

That there shall be financial assistance to any active member of

AVSEGROUP, whether uniformed or non-uniformed personnel, who
encountered serious injuries that creates a substantial risk of death or that
causes prolonged injury or loss or impairment of the function of a limb or bodily
organ such as:

 - Traumatic brain injuries;

 - Spinal cord injuries that result in partial or total paralysis;
 - Injuries that result in the loss of an arm, leg, eyesight, or hearing;
 - Injuries that result in permanent damage to an organ or loss of function of
 an organ or significant limitation on the function of an organ;
 - Severe burn injuries; and
 - Fractures and dislocations: head, back, chest and abdomen, neck, hip
 and pelvis.

 The amount of Php25,000.00 will be provided as financial assistance;

That the AVSEGROUP “Pagtutulungan” Project is not a mandatory contribution

to all, only personnel who are willing to support the said project will contribute Php50.00
to support the funding to the AVSEGROUP “Pagtutulungan” Project and No contribution
shall be made until the whole collected amount is not spent to those who are in need.

That once the signatory to this affidavit of contribution was transferred to other
unit outside AVSEGROUP, he/she is still entitled with the benefits to the AVSEGROUP
“Pagtutulungan” Project.
That my fellow Police Officers who can avail of this financial assistance should
also be an agreeing party to this Affidavit of Contribution;

That we are aware that this contribution is being made gratuitously without any
exchange of monetary or other actual consideration herein;

That the main purpose of our contribution is purely an act of magnanimity for the
spirit of brotherhood, camaraderie and not for any onerous consideration;

That we are executing this affidavit to attest freely and willingly the veracity of the
foregoing circumstances;
