Dear Puteri Nurul Balqis Binti Mohd Razi @ Rozi ,

OFFER OF CONTRACT OF EMPLOYMENT


We are pleased to offer you a permanent contract of Employment on the following terms and conditions:

DESIGNATION : CREW - JUNIOR

BRAND : Gigi Coffee

WORK LOCATION : AEON MALL Tebrau City

RELOCATION : Company reserves the right to transfer you to other outlet throughout
the entire duration of your work here even if the existence of such
outlet only happens after this contract is made.

COMMENCEMENT DATE : 05 Jan 2023

WORKING HOURS : 7.5 working hours between 9am to 10:30pm


(excluding 1.5 hours break)
6 working days & 1 rest day per week

BASIC SALARY : RM1800

EMPLOYEE PROVIDENT : Full Employer’s and Employee’s EPF contributions according to the
FUND (E.P.F), SOCSO & prevailing EPF Act and all other applicable and mandatory statutory
EMPLOYMENT INSURANCE contributions.
SYSTEM (E.I.S)

INCOME TAX & : You shall be liable for your own Personal Income Tax. If you are leaving
RETIREMENT AGE Malaysia permanently or due to termination of employment upon
retirement age of 60 years old, the Company shall withhold any money
due to you until clearance has been granted by the income tax
authorities.

ANNUAL LEAVE : You will be granted eight (8) days on a prorated basis in a year.

MEDICAL LEAVE : On recommendation of a Certified Medical Practitioner, you will be


granted fourteen (14) days of paid medical leave in each year.
Where hospitalization is required, your total medical leave entitlement
shall be extended to a maximum of sixty (60) days in any one calendar
year.

MEDICAL CLAIMS : You are entitled to claim outpatient medical expenses up to RM900 per
year upon Management’s approval. You will be responsible for any
excess medical cost should the medical expenses exceed your
coverage limits.

PROBATIONARY PERIOD, : At any time during the period of probation (either initial or extended
TERMINATION NOTICE & as the case may be, or on the expiration thereof) or upon your
TERMINATION RELATED confirmation of employment, your employment may be terminated
MATTERS either by the Company or by yourself, by serving to other party,
written notice or by payment of your salary in lieu of such notice,
without assigning any reason whatsoever for such termination.

Upon cessation of your employment with the Company, you shall


return all properties belonging to the Company (including, but not
limited to, uniform, locker keys, laptop and other documents,
confidential or otherwise obtained or made by you during the course
of your employment with the Company, pertaining to the business of
the Company.
However, the above said period of notice or quantum of indemnity in
lieu of notice may be reduced or waived, only by mutual agreement
between yourself and the Company.
Notwithstanding anything to the contrary herein before stated, the
Company may at its sole discretion without due notice or payment in
lieu of such notice terminates the contract of employment immediately
upon (but not limited to) the happening of any of the following events:
a) If you constantly display a performance that is below the
acceptable level.
b) If you shall enter into any arrangements with creditors that will
interfere / hinder your work performance or cause disruptions
to the Company.
c) If you shall at any time be declared a bankrupt during your
employment.
d) If you shall be guilty of any misconduct inconsistent with the
expressed and/or implied conditions of service or willful
neglect of duties.
e) If you are addicted to any habit which renders it unsafe or
unwise for the Company to continue to employ you.

NON-DISCLOSURE & : You will not divulge, directly or indirectly to third parties any
CONFIDENTIALITY confidential operations, processes or dealings, any trade secrets or any
information concerning the business or affairs of the Company, during
and after the period of your contract of employment with the
Company.

OTHER TERMS & : The other terms, conditions and benefits of employment shall be in
CONDITIONS AND accordance with the relevant sections(s) of the Staff Handbook. The
BENEFITS Management reserves the right to amend, add or delete the contents
from time to time without any prior notification to you.

We wish to emphasize that this offer of Employment is conditional based on the fulfillment of the following
requirements: -

a) Accurate submission of your personal data and information in both of your job application form and job
interview, and
b) Completion of Covid-19 vaccination with the following recognized dosage by the Ministry of Health (KKM):
-

i) 1 dose - Johnson & Johnson, CanSino


ii) 2 doses - Pfizer, AstraZeneca, Sinovac
Please acknowledge receipt of your acceptance of this offer of Employment by signing and returning the
duplicate of this letter to us within three (3) days from the date of this letter, failing which, this offer of
Employment shall lapse.
