NCM 106 – Related Learning Experience


PERFORMANCE RATING SCALE – GIVING INJECTIONS

GENERAL PROCEDURES FOR ADMINISTERING INJECTABLE MEDICATIONS ASSESSMENT:

1. Review and examine MAR (Medication Administration Record) or Therapeutic sheet for
accuracy and completeness.
2. Review information from drug handbook/PIMS/MIMS about the medication to be
administered.
3. Assess patient’s abilities including size and build.
4. Assess the need for assistance.

5. Assess patient’s PRN medications in case of side effects.


PLANNING:

6. Determine and prepare material needed.


7. Determine appropriate needle and syringe to be used.

8. Wash hands.
IMPLEMENTATION:

9. Read name of medication from the patient’s record.


10. Check the label on the medication before picking it up.

11. Check the expiration date, color and consistency of medication.


12. Remove correct amount of medication after calculating the correct dose.

FOR VIALS:

a. Clean top of vial using cotton balls with alcohol or alcohol swab and allow
drying/use dry cotton balls.
b. Discard used cotton balls and alcohol swab.
c. Draw air into syringe equal to amount of medication to be withdrawn.

d. Insert needle into vial through rubber stopper.


e. Inject air into the vial.

f. Pick up vial with non dominant hand and withdraw correct volume of medication.
g. Examine for air bubbles and expel them.

h. Recheck volume of medication for accuracy.


i. Remove needle from vial.

j. Change aspirating needle with new one.


k. Replace needle cap.
FROM AMPULES:

a. Clean neck of ampule with cotton balls with alcohol or alcohol swab.
b. Wrap neck of ampule and break off the top, away from yourself.
c. Remove needle guard.

d. Hold ampule in non dominant hand and insert needle into ampule.
e. Aspirate medication into syringe.

Right dose, Right time, Right route, Right of the patient to refuse, Right assessment,

Right education, Right evaluation, and Right documentation.

FOR INTRAMUSCULAR INJECTIONS:

1. Determine and assess site for injection. Use deltoid muscle/other IM site.
2. Disinfect the area in a circular motion and allow drying. Repeat if needed.
3. Spread skin with thumb and index finger.

4. Ensure the needle is 90° angle.


5. Insert the needle slowly and aspirate for blood. If you aspirate blood, withdraw
immediately the needle. If there’s no blood aspirated, slowly inject the drug.
6. Withdraw needle and put dry cotton balls and massage the site.

7. Assess for side effects or adverse reactions after 30 mins.


FOR SUBCUTANEOUS INJECTIONS:

1. Pinch skin between thumb and index finger to elevate subcutaneous tissue.
2. Use 45° to 90° angle.
3. Insert the needle slowly and aspirate for blood. If you aspirate blood, withdraw
immediately the needle. If there’s no blood aspirated, slowly inject the drug.
4. Withdraw needle and slightly wipe with dry cotton and NO massage on the site.

5. Assess for side effects or adverse reactions after 30 mins.


FOR INTRADERMAL INJECTIONS:

1. Stretch skin at selected site.


2. Hold syringe at 10° to 15° angle, with needle bevel facing up.

3. Insert needle just until bevel is no longer visible.


4. Inject medication slowly until formation of a bleb/wheal.

5. Do not massage.
6. Circle site with a marking pen (black or blue pen but NOT RED PEN). Then mark
the due time/date.
7. Withdraw needle.

8. Assess any untoward reactions after 30 mins.


16. Position patient comfortably.

17. Discard materials used.

DOCUMENTATION:

18. Document the medication that was given, including the name of medication, dosage,
route, time given and your signature.
