AFFIDAVIT OF UNDERTAKING

I, Judie Anne Villanueva of Legal age, with office address at 3F Metro Lifestyle Complex,
Corner F. Torres E. Jacinto Ext., Davao City Philippines after duly sworn in accordance with
the law, do herby depose and state that;

1. That I am the Supply Chain Manager and Responsible Officer of UPL PHILIPPINES
INC.
2. That our shipment 2X40 STC VONDOZEB 42SC arrived at the port of Davao from
Vietnam last November 26, 2022 on board KOTA HAPAS 0371E with Registry No.
PIL0017-22 covered with BL No. SGNCB22021619.
3. That we are requesting for immediate release of the subject shipment pending
submission of the Original Document due to delayed transmission from origin to our
end.
4. That we promise to submit the aforementioned signed original copy of Invoice and
Packing List once available.
5. That we hold the government of the Philippines free and harmless from any
consequence that will arise due to this request.
