FUNDAMENTALS OF NURSING PRACTICE-LECTURE

Give an example of assessment in the nursing process at least 5 examples each. (all
types of assessment)

1. INITIAL ASSESSMENT
● Marielle complains about her cough which is about 7 days already. She was
interviewed and assessed by a nurse regarding her complaint and the nurse
documented and gathered all the data so that he/she can start making a
diagnosis about it.
● Jean complains about having difficulties with hearing. The nurse
interviewed her and do an ear assessment. The nurse documents the
findings and gives them to the otolaryngologist.
● The client complains about his/her sudden headache and blurry vision. The
nurse interviewed him/her regarding his/her eyes and other symptoms that
he/she feels. The nurse takes note of it and does an eye assessment of the
client.
● Kylie complains about her back pain which is about 4 days already. The
nurse interviewed her about it and documented all the data that she
gathered.
● Ms. Velasquez complains about her severe headache. The nurse interviewed
her and found out that she had a car accident before. The nurse does the
skull and face assessment to know if there are any abnormalities that are
needed to be assessed immediately.
2. PROBLEM-FOCUSED ASSESSMENT
● The nurse does an X-ray of the patient who had a motor accident.
● Nurse Kyle assessed and monitored the client’s platelet.
● Nurse Marielle does an eye assessment of the client who’s complaining
about his/her blurry vision.
● The nurse assessed the client that is complaining about chest pain.
● Nurse Jean does a skin assessment to the client who’s complaining about
his/her skin allergies.
3. EMERGENCY ASSESSMENT
● The nurse immediately assessed the client that had severe bleeding caused
by a car accident.
● The nurse assessed Mrs. Claros's consciousness during the seizure attack.
● Nurse Kyla assessed the client that had trouble breathing caused by an
asthma attack.
● Nurse Johann assessed Kylie immediately who passed out and hit her head
on the floor.
● The nurse assessed immediately the son of Mr. Chua after swallowing the
marble.
4. TIME-LAPSED ASSESSMENT
● After 6 months, the nurse re-assessed the patient’s mobility status after
having surgery for a bone fracture.
● After an hour, the nurse repeats the assessment of the client’s platelets
blood test that has dengue.
● After 8 months, Kyla is being re-assessed by Nurse Maye regarding her
mental health status after being diagnosed with depression
● The nurse did the interview with the client after 1 week of being
hospitalized because of over-fatigue.
● After 2 months, Nurse Kyle re-assessed the patient’s eye vision for having
astigmatism.
1. How do you write a nursing diagnosis assessment? Write an example.
● The nursing diagnosis assessment is written using terminologies. NANDA is
offered in order to have a common language in making a Nursing evaluation. This
is written in the following manner: Problem/diagnostic label “related to”
etiology/related factors “as evidenced by” defining characteristics/ risk factors
● For example, Excessive fluid volume related to congestive heart failure as
evidenced by symptoms of edema.

2. What is the relationship between nursing assessment and nursing diagnosis?


● The relationship between the two is that they are both in the nursing process which
indicates that if there are no nursing diagnoses if there is no nursing assessment
since nursing assessment is performed to collect all subjective data verbalized by
the patient as well as objective data observed, assessed, and measured by the nurse
while nursing diagnosis is derived from all of the data gathered from the patient
throughout the nursing assessment.
How do nurses demonstrate the assessment step of the nursing process?
● First, they will perform hand hygiene for infection control and provide privacy for
the patient to make them comfortable. Then, the nurse will verify the client’s
identity and explain the technique and why it is necessary, as well as conduct an
interview or take a history of the client's major complaint to acquire subjective
data. Physical examination and observation of the customer yield objective facts.
Finally, summarize or restate the collected data and describe the findings.
Why is the assessment step of the nursing process important?
● It is the first step in the nursing process and it is important because it serves as a
baseline data that makes the nursing process to be accurate and effective.
What are some examples of an effective assessment?
● The patient with severe headache was assessed by the nurse. The nurse were
gathered all the data that will serve as her baseline and make suitable care for the
patient. Then, the patient becomes satisfied because she has been cured and had
been take care of.
