KJGY/TKD/E BCACBM at GZB due to repeated dis-coupling on dated 18.03.2023 Brief of incident: The train no. KJGY/TKD BCACBM load of 27+2 (1 DBKM+1 BVZI) wagons was arriving towards GZB, after its ROH (from JUDW) with End-to-End BPC (from KJGY/UMB) without any previous reported abnormality. On-board crew (at time of incidence) - LP and ALP (Crew change at GZB), Load was running without Guard. 1. After stoppage at GZB home (16:08-17:40 hrs) - upon starting, LP observed sudden pressure drop and upon rake examination ALP found BP hose pipe dis-coupling between 2nd & 3rd wagons (from engine). 2. After re-coupling, upon start the load was reported to dis-couple again twice at 17:52 and 17:56 hrs (after re-couple by crew themselves, the LP started at 18:01 hrs and cleared block section at 18:07 hrs). TXR staff was given info/ memo by CYM at 18:10 hrs (no info through sectional controllers to C&W control), arrived on location i.e. Mainline no. 7 at 18:25 hrs and after examination gave FIT at 18:40 hrs. No problem/ abnormality reported from thereafter till arrival at destination TKD. Shri Krishan HQ/GZB Guard Without guard | Type of stock BCACBM Investigation: Following aspects were observed:- ‘SN | Parameter/ Aspect Status/ Remarks 1. | BP hose pipe free length (2nd & 3rd wagons | Observed Lengths = 750 mm from Engine) (each) - (standard 655 +/- 6 mm) 2. | Condition of sealing wire, MU washers Good 3._| Angle cock orientation Proper (i) The BP hose pipes were found to be of standard make/ sufficient length. There was no pre-stress/twist in hose pipes and orientation of Angle cocks was also Proper. Moreover, there was observed sufficient slack i.e. looseness in the pipe-to- pipe connection to allow for curve negotiation. (i) Furthermore, it is noteworthy that the train arrived from KJGY running at full speed through various curves en-route without any previous reported abnormality. hrs halt near GZB, some miscreant activity resulted into Fea ae ice pipes and upon lond start caused dis coupling for first ime. Tt was observed that there was no sealing wire to tie-up the 2 hose-pipes together (b/w any two wagons except the dis coupled pair), KJGY depot which issued End- 7 to negotiate curves at End BPC for this load is advised that the load hi at n-route/ has various halt points such as GZB (in this case) all hose pipes should be tied up with cross Further notwithstanding above, the ALP failed to re-couple BP hose pipes properly & tie-up in-tactfully, which resulted into further dis-coupling two times. Thereafter, once the coupling was properly set, no more abnormality was reported in rake till it reached destination. Therefore the crew is responsible for failing to ensure proper coupling of BP hose pipes. Remarks: ~ (i) The speedometer readings for said duration haves been called for (ii) The on-board crew i.e. LP and ALP have been called for examination.
