    This Employment Contract ("Contract") is made and entered into on [DATE] by and

    between [EMPLOYER NAME], a [STATE] corporation, having its principal place of


    business at [ADDRESS] ("Employer"), and [EMPLOYEE NAME], residing at [ADDRESS]
    ("Employee").

    POSITION. Employee shall be employed by Employer in the position of [POSITION]


    ("Position").

    TERM. This Contract shall begin on [START DATE] and shall continue until terminated
    by either party on [NOTICE PERIOD] days' written notice to the other party.

    DUTIES AND RESPONSIBILITIES. Employee shall perform all duties and responsibilities
    as may be assigned by Employer, including but not limited to [LIST OF DUTIES].
    Employee shall devote full time and attention to the business of Employer and shall
    use best efforts to promote the interests of Employer.

    COMPENSATION. In consideration of the services to be performed by Employee,
