or electronic PDF copy you can do so by completing and returning in one of the following ways.

Electronic – First, save the blank form to your hard-drive, complete the form and click ‘save’. Then you can email the saved file
to anitathew@adventist.org.au

Hard copy – Either save and complete as per the electronic instructions above, or print the blank form, fill out manually and post
the printed form to: Seventh-day Adventist Church, Administration & Resourcing Centre, PO Box 577, Spring Hill, QLD, 4004.
