GUARANTEE LETTER PROCESS AND REQUIREMENTS

I. PCSO

Requirements:

1) Medical Abstract (Nurse Station)

 Must be a Certified True Copy (“CTC”)
 Must bear the license number of the attending physician
 Must be signed by the attending physician

2) Statement of Account Summary (In House Collection)

3) Identification Card (“ID”) of the patient

4) ID of the patient’s relative

Process:

1) Scan all requirements

2) Fill-out online application and submit scanned copy of the requirements to
https://www.pcso.gov.ph/webapps/NCROnlineApplication.aspx from 8AM to 9AM every
Monday to Friday

II. DSWD

Requirements:

1) Medical Abstract (Nurse Station)

 Must be a Certified True Copy (“CTC”)
 Must bear the license number of the attending physician
 Must be signed by the attending physician

2) Statement of Account Summary (St. Luke’s In-house Collection Department)

3) Referral Letter (St Luke’s Social Service Department)

4) Case study from local Social Welfare and Development Office

5) Certificate of Indigency

Process:

1) Submit all documents the Department of Social Welfare and Development Central Office
located at DSWD Building, Constitution Hills, Batasan Complex, Quezon City, PH 1126
