AFFIDAVIT OF DISCREPANCY

I, ISABEL GUINDALOS BULICDAY, of legal age, single, Filipino citizen and a

resident of FC-292 Central Balili, La Trinidad, Benguet under oath depose and say that:

1. I was born on April 15, 1962 at Balili, Bontoc, Mt. Province to spouses Loreta
Guindalos and Sebio Bulicday;

2. Said facts were evidenced by my Certificate of Live Birth with Registry No. 2007-
936 registered at Bontoc, Mt. Province;

3. In my records at the Social Security System, it appears that I have two personal
records, to wit:
First Record:
a. Name: ISABEL GUINDALOS BULICDAY
b. Birthdate: APRIL 15, 1962
c. Mother’s Name: LORETA GUINDALOS
Second Record:
a. Name: ISABEL TABCAO BOLICDAY
b. Birthdate: SEPTEMBER 15, 1966
c. Mother’s Name: MARRY TABCAO

4. My true and correct records are indicated on my first record, to wit:

Name: ISABEL GUINDALOS BULICDAY

Birthdate: APRIL 15, 1962
Mother’s Name: LORETA GUINDALOS

5. I appeal to the concerned agency to cancel my second record and honor the first
record as it is my true and correct records;

6. The discrepancies in the foregoing entries were not intentionally done or made for
any unlawful purpose;

7. I am executing this affidavit to attest to the veracity of the foregoing facts and for
whatever legal intents and purposes this may well serve.
