(6) Objective(s) of the Activity:


1. To vaccinate target eligible population 6-59 months for Measles-Rubella/ Measles,
Mumps and Rubella, 9-59months for Japanese Encephalitis Vaccine, 0-59 months for bOPV and
provide the appropriate dose of Vitamin A as necessary to eligible population given measles-
containing vaccine.
2. Conduct health education and advocacy on Opan Culex and Project Baby Come Back
To Bakuna.
3. Prepares and submits accomplishment on vaccinated target eligible population.
4. Prepares and submits report of home visits, followed up patients and community
activities conducted.
5. Coordinate with appropriate District Health Center personnel assigned in his/her area
to ensure timely data collection and reporting.
6. Perform other functions as necessary.

Learning Insights:

This month is full of excitement because of the activities that has been done. One learning
outstands is the importance of cooperation and unity. I learned that positive result would be achieve
when everyone work hand and hand in reaching the goal. Also, It’s not just the high number of
children vaccinated is what we call achievement or accomplishment but also the appropriate
assessment and implementation rendered.

(8) Summary of issues, actions done and recommendations/agreements:

So far, I don’t have any issues and concerns regarding my exposure to my assigned areas. The
success of this program launched by the government may rely on the sustenance and continuation of
the services being offered by DOH. And this necessitates us, nurses, to commit ourselves in performing
whatever is expected of us. Therefore, I recommend to continue and maintain this kind of activities
and to conduct more actions to promote good quality of health services.
