PARTICIPANTS WAIVER AND CONFORME

I, __________________________, participating in JPICE – LNM General

Assembly & Acquaintance Party 2023 entitled: “TransCEnd: Lead from
Within” which will be held at Patio de Manila located at Bay Park, 1879 M.
H. Del Pilar St, Malate, Manila on April 2, 2023. This event will take place
under the guidance and direction of the organizers – Junior Philippine
Institute of Civil Engineers - Lungsod ng Maynila (JPICE-LNM), recognize
the probability of risks that come with my attendance to the said event.

Despite the risks, I recognize, understand, and take full responsibility of

any action of my own doing which may cause harm upon my person. I
also give my full confidence to the organizers, its officers, directors and
agents, employees, faculty, or representatives associated with the
event, and trust them not to cause any kind of harm and prevent any
eventuality of such to happen to me.

I hereby agree to hold harmless and indemnify the organizers from all
actions, claims, demands, damages, costs, expenses, and all
consequential damage arising from or in connection with my
attendance to the said event or in connection with any illness or injury
or cost of medical treatment in connection there with.

In signing the Waiver, I am not relying on any oral or written

representation or statements made by the organizers or representatives
associated with the event, to induce me to take the trip, other than
those set out in this Waiver.
I also agree and confirm that:
a) I am not below 15 years of age and has my parent’s consent to
participate;
b) The information I have supplied during registration are correct to
the best of my knowledge;
c) My participation in JPICE – LNM General Assembly &
Acquaintance Party 2023;
d) The JPICE – LNM General Assembly & Acquaintance Party 2023 is
a no-smoking-and-no-vice zone and I will not engage in any of
these behaviors for the duration of my stay. For the Acquaintance
Party where drinks are expected, I’ll held myself responsible and I
will abide to practice Social or Moderate drinking;
 e) The JPICE – LNM General Assembly & Acquaintance Party 2023
and its Project Executive Organizers will not be responsible for lost
personal items that I did not surrender to them for safekeeping;
f) I have read and understood the terms of this consent and waiver
and its attachments, and recognize that it is binding upon me, my
heirs, executors, and administrators upon my signature.

For any concerns and/or questions about the JPICE – LNM General
Assembly & Acquaintance Party 2023, it is expected of me to get in
touch with the Project Executive Organizers.

This is also to confirm that I have signed this Participants Conforme

Sheet of my own will with the understanding that failure to follow the
process outlined herein shall be construed as lapse on my part and
may result in my not being allowed to participate in the JPICE – LNM
General Assembly & Acquaintance Party 2023.

 PARENT’S CONSENT FORM

The nature and the purpose of the JPICE – LNM General Assembly &
Acquaintance Party 2023 have been explained to me through the
document that my child received.
We understand that participating in this program is entirely voluntary.
We have agreed to have my child participate in the program on
April 2, 2023, at Patio de Manila located at Bay Park,
1879 M. H. Del Pilar St, Malate, Manila.
We understand that due to the program of JPICE-LNM General
Assembly and Acquaintance Party 2023, my child’s mobile phone may
be turned off and cannot be reached. We agree to release the Junior
Philippine Institute of Civil Engineers - Lungsod ng Maynila (JPICE-LNM),
its officers, and adviser, from and against any, and all liability, loss,
damages, claims, or actions, to the maximum extent permissible by law,
arising out of the said event. We agree that my child may be
photographed, filmed, interviewed, or otherwise identified by the
media. We also agree that my child’s personal information can be
collected, used, and shared by the Junior Philippine Institute of Civil
Engineers - Lungsod ng Maynila for purposes within the bounds of the
law.
