A. Original Conditions of Probation


1. Probationer shall report initially to the Chief Probation and Parole Officer at
Tuguegarao City Parole and Probation Office within seventy-two (72) hours
from receipt of the order granting probation;
2. He shall, thereafter, report to his supervising Probation and Parole Officer
at least once a month unless otherwise modified by the Chief Probation and
Parole fficer;
3. He shall reside at Malama, Conner, Apayao and shall not change said
residence without prior approval of the Chief Probation and Parole Officer,
as the case maybe;
4. He shall secure a written permit to travel outside the jurisdiction of
Tuguegarao City Parole and Probation Office, from the Chief Parole and
Probation fficer, and from the court if such travel exceeds thirty (30) days;
5. He shall not commit any crime or any other offense;
6. He shall render community service and will participate in tree-planting
activities;
7. He shall render at least fofi-eight (48) hours of community servicc;
8. He shall plant and maintain at least twenty-five (25) fruit bearing trees during
the period of probation;
9. He shall allow the supervising Probation and Parole officer or an authorized
Volunteer Probation Aide to visit his home and place of work;
10. He shall meet his family responsibilities;
11.He shall undergo medical, psychological or psychiatric examination and
treatment and enter and remain in a specffied institution when required for
that purpose;

't 2. He shall devote himself to a specific employment and


shall not change said
employment without prior notice to the Supervising Officer and/or pursue
prescribed secular study or vocational training;
13. He shall refrain from associating with persons of questionable character;

14. He shall undergo random Drug Testing every six (6) months or as required
by the Supervising Officer or Chief Probation and Parole Officer to rule
possible drug use during his probation period and be subjected to Drug
Dependency Examination if confirmed positive;
15.He shall cooperate with his program of supervision and shall satisfo any
other condition related to his rehabilitation and not unduly restrictive to his
liberty incompatible with his freedom of conscience'
16.The probationer shall undergo his probation for a period of THREE(3)
YEARS to be counted from the probationer initial report for supervision.

B. Other Conditions: None


C. Program of Supervision:

Probationer must satisfactorily comply with his probation conditions. His case was
classified under medium supervision. He was required to report once a month and to
attend the Therapeutic Community Ladderized Program and participate in the various
programs and activities like Tree Planting, Community Work Service, Skills Training and
General Assembly of clients and other activities which required his participation.

D. Response of Probationer to Supervision Program

Probationer presented himself to this office for his initial supervision on July 05,
2019. Conditions of his release on probation were explained to him very well which he
fully understood and accepted. He was required to report personally to this office once a
month and thereafter which he did regularly.

During probationer's supervision period, he religiously attended all programs and


activities of the office geared towards his development and reformation. He was observed
to be attentive, cooperative and committed. His attendance to the sessions and activities
made a significant impact on client's behavior as he was able to improve his attitude and
behavior in dealing with people and use to handle problems that may crop-up along the
way. \Mth constant supervision, his learningswhile on probation had manifested a socially
acceptable behavior in the community where he belongs.

Probationer had improved his inter-personal relationship and developed his social
responsibilities through his participation in the different reinforcing activities like physical
fitness, linis-bayan, team-building activities and leadership seminars. He was also made
to undergo random drug testing and was found negative on all occasions.

Restorative Justice Program was also instituted upon him where he accepted his
wrongdoings and misconduct and felt remorseful thereafter. By accepting his mistakes
and as a way of integrating himself to the community, he was able to render forty-eight
(48) hours community work service. Home visits conducted in Gagabutan East, Rizal,
Cagayan where is stayed during the period of his probation and information gathered
from his family, neighbors and barangay officials revealed positive responses on

probationer's behavior and attitude. He was cooperative and supportive to the programs
of the barangay and had become a more responsible and law-abiding person.

Probationer has neither violated any of the conditions of his probation nor
commifted any other offense during the period of his probation.

ln view of the reformed behavior manifested by the probationer, this office


confidently declares that he is now ready to be reintegrated into the mainstream of society
and that extension of his probation period is unnecessary.

E. RECOMilIENDATION:
ln view of the foregoing and considering that probationer satisfactorily complied
with all the conditions of his probation and is now leading a reformed life, it is most
respectfully recommended to the Honorable Court the official termination of the probation
granted to JULIUS LAGUA y CAUILAN immediately upon approval of this report and
that all civil rights lost or suspended on account of his conviction be restored.
