WAIVER/ AGREEMENT
I hereby affix my signature to confirm that I am allowing my son/daughter to join the “LEADERSHIP
ENHANCEMENT” which will be held on February 24-26, 2023 in Vista Pamplona, Nagattatan, Pamplona,
Cagayan.
I am not holding the school responsible for whatever violations he/she commit and for any situation beyond the
control of the Leadership Camp team/organizers in the said activity provided that:
1) Time management should be strict practiced
2) There will be responsible adult members of the team (teachers/advisers) to look after the need of my
son/daughter.
3) That all the undersigned voluntarily agree and consent to the covenant given by the school;
4) While there remains the risk of possible COVID-19 transmission to my child, and to the members of my
household, I freely assume the said risk and I permit my child to attend school practices under this activity
with the condition that my child is already fully vaccinated.
5) That all the undersigned voluntarily agree and consent to the covenant given by the school;
6) That we all waive, renounce any liability or obligation against MCNP - ISAP for any injury, incapacity or
death occurring during the activity; and
7) The students are well oriented before the conduct of the activity on the observance of the following camp
rules:
- To observe Cleanliness and Orderliness inside the camp and school buses.
- To protect the environment and school facilities from any damage.
- The students must wear the prescribed attire (pants, t-shirt and rubber shoes) for their safety and comfort.
- Bringing or drinking of alcoholic beverages, prohibited drugs, smoking, and bringing of deadly weapons are
strictly prohibited.
- Students are not allowed to go out of the vicinity of the camp or leave during and before the end of the
activity except for emergency purposes and with the permission of the teachers or the leadership camp
organizers.
- Students are not also allowed to go beyond the playground and anywhere else inside the camp that is not
necessary to be visited.
- Students are held responsible for their wallets, jewelries, cell phones, and other valuable items.
- Students are instructed to bring:
 Jacket and hat
 Ball pen and writing journals
 Extra clothes
 Beddings (Pillow, Bed sheet, Blanket)
 Toiletries
 Medicines
 Umbrella
 Protection against insect bites
 Eating Paraphernalia
8. That we are executing this waiver/ agreement freely and voluntarily.
Note: The leadership camp organizers reserve the right to carry out other rules and regulations as stipulated in the
student’s handbook for any violation/stipulated that is beyond their control.

WAIVER

In this regard, the school and the teachers-in-charge are not liable for any untoward accident that is beyond their
control.
