PARTICIPANT'S CONSENT

I, (Jan Carlos G. Sunga), of age (19), hereby informed to attend the (Mr. and Ms. PLP Rehearsal).

Any danger that may arise from my irresponsible actions or failure to follow instructions, from broken tools, equipment,
instruments, and other property possessed, regulated, and managed, or because of unintentional circumstances that might
affect it, will make me responsible.
 
● I CERTIFY that I am healthy, in-shape, do not have diseases that might harm other people, am able to perform the
activity, have enough preparation or training to cooperate in the activity, and am not affected by serious medical
conditions or hindered from cooperating by a medical professional.

● I ACKNOWLEDGE that this Waiver will be used by the Student Council, Organizers and Officials of the activity
in which I have attended, and it will cover my rightful act and authority at the event. In consideration of
accepting, allowing and giving permission to attend the event, I hereby think and use ethical actions for myself,
authorities, administrators, student council, and fellow students.

● I HEREBY DECLARE UNINVOLVEMENT of Pamantasan ng Lungsod ng Pasig and all the officers and
authorities in the liability and accountability for possible injuries, illnesses, accidents, and natural disaster during
the performance in the event that I participated.

● I RELEASE any liabilities from the said authority but there are limitations in terms of, liability from the
negligence, mistake or uncareful action of entities or authorities, for my death, disability, personal injury, damage
of property and any kind.

● I PROMISE NOT TO APPEAL to the parties or persons involved in the waiver. All liabilities or claims including
the participation of this event or whether caused by the negligence of release or otherwise.

● I CERTIFY THAT I CAREFULLY AND COMPREHENSIVELY READ THIS DOCUMENT, AND I TRULY
UNDERSTAND THE CONTENT OF THE WAIVER. I AM AWARE OF THE CONDITIONS AND RELEASE
OF LIABILITY. I SIGNED IT IN MY OWN FREE WILL AND PERMISSION.

PARENT'S CONSENT

I, (Estela G. Sunga), of legal age, allow my daughter/son to attend the (Mr. and Ms. PLP Rehearsal),
Name of Parent/Guardian Activity Title
on March 5 and 12, 2023, from 8:00 a.m. to 5:00 p.m. at PLP Function Hall, and on March 14 and 15, 2023, from 8:00
a.m. to 5:00 p.m. at the PLP Main Building 7/F. I agree on behalf of my daughter/son with the release of liability and will
not make the parties above-mentioned free from liability for harm, accidents, damage, or expenses.
 
● I do not allow my daughter/son to join the activity, but will comply with the alternative academic activity.
