Assignment Description:

The purpose of the Design & Planning phase of the Community Connection Program is to thoughtfully
design a community-based program with a music component that you can implement to enhance your
personal and/or community’s health and/or wellness.

Instructions:

Step 1: Complete the Program Design Template for this specific assignment

Step 2: Attach a written summary that includes a detailed description of the following:
Why you have chosen to create a program for your chosen participant(s)

Why your personal goals are important to you

Your detailed communication plan for inviting your participant(s) to be a part of your program. You may
include draft emails, texts, or other types of correspondence.

Expectations:

Upload two word and/or PDF documents to the applicable assignment section of MyCanvas:

1) Program Design Template

2) Written Summary (see above for expectations)

Alternatively, you may amalgamate the two documents into one for submission.
