AFFIDAVIT OF CANCELLATION
WITH NO FINANCIAL OBLIGATION

KNOW ALL MEN BY THESE PRESENTS:

I, LENIE MAY BUENO BACALA, of legal age, Filipino, single, and resident
of BRIQUE PLAZA PALICO IV 4103 IMUS, CAVITE, after having been sworn in
accordance with law, hereby deposes and states the following:

1. That I am registered owner of a business under the trade name AIM-EL

BILLS PAYMENT CENTER located at IMUS, CAVITE;

2. The above-stated business was duly registered at the Department of

Trade and Industry (DTI) on 29th day of March 2021;

3. At present, I am causing the cancellation of the said registered

business owner due to no financial obligation or no income;

4. As far as my personal knowledge as the registered owner, I attest to

the fact that LENIE MAY BUENO BACALA has no outstanding
financial obligation to any individual, public or private corporation
or entities.

5. That I am executing to this affidavit to attest the truthfulness of the

foregoing declarations under penalties of perjury to respectfully request to
DTI to CANCEL the registered business owner LENIE MAY BUENO BACALA,
and for other legal purposes.
