hereby declare that I have been contributing PF i.e., wage ceiling Rs.5000/- (01.10.1994 to
31.05.2001) w.e.f . NIL
hereby declare that I have been contributing PF i.e., wage ceiling Rs.6500/- (01.06.2001 to
31.03.2008) w.e.f . NIL
hereby declare that I have been contributing Provident Fund on my entire salary @ 12%
with effect from:06.03.2009.
I am/am not an 'EXCLUDED EMPLOYEE' within the meaning of para 2(1) of the EPF
Scheme, 1952.

I request that;
i. I may be enrolled as member of the Employees' Provident Fund voluntarily with
effect From. 06.03.2009.
ii. I may be permitted to contribute voluntarily on my entire salary exceeding
Rs'6500/- or Rs. 15000/- per month w.e.f. 06.03.2009.
iii. I may be permitted to contribute @ 12% instead of the statutory rate of 12%
with effect from 06.03.2009.

5. We have paid Administrative Charges and submitted all the Returns in respect of the
above Member accordingly and will continue to do so.

We request that this case be regularized by permitting voluntary membership and contribute on
entire salary @ 12% of pay as stated above.

Payment to my EPS account has been maintain with RPFC, Guntur through my employer.

2) That, I also undertake that in case my balance in PF account is insufficient to meet the aforesaid
requirement, I am ready to pay the differential amount to RPFC, Guntur thus making me eligible
for drawing pension on the basis of actual salary/wages instead of on the salary/wages as per
prevailing ceiling under the act.

3) That, I understand that the above declaration is made to opt for higher pension on higher salary
as per the honorable Supreme Court ruling dt.04.11.2022 with respect to the Employees’
pension scheme, 1995.
