NARRATIVE REPORT
The target family was one of the families in our barangay neighborhood (San
Antonio Pob. Nabua, Camarines Sur). It was easy for me to coordinate with them. First, I
asked for the willingness of the family for the home visit. I planned the flow of the whole
process and prepared the information forms that I will be using for the interview. The client
and I agreed upon the date of their availability for the visit.
On the day of the visit, (December 10, 2022), I asked politely the client letting me
inside the house. I greeted the family while I asked the permission to sit. I then established
rapport by talking with the client and joking with the youngest child, John Enzo. Then, I
stated the purpose of the visit by letting them know that I am a Nursing Student from
CSPC completing a project in Community Health Nursing. I stated that I will have to profile
the whole family and all the family members and also their current living condition. I
proceeded to the interview about the health history of the family, the used the form
throughout the rest of the interview. I made sure to tell the client that all information to be
shared is going to be confidential and private. After noting all the problems that could
cause health problems especially to the children, I thoroughly explained to the mother the
importance of a well sanitated environment, the possible consequences of lack of
cleanliness for their health. I educated the client about proper waste disposal and
segregation.
After the interview, I then thanked the family for extending their time for a home
visit. I documented the events that transpired throughout the visit.
