We, the group 4 researchers of 12 STEM St, John would like to ask permission to access some of
the private information of the students of Grade 10 students, Bethlehem, Nazareth, and Galilee of
La Salette of Roxas College. This information includes the Names, Age, Religion, and Gender.
This is information will be used in our research entitled, “Effects of Participation in the Holy
Eucharist to the Grade 10 Students. This will be in the part of the demographic group in the
sample of our research population.
In addition, we would like to ask permission to grant us access in having the names of the
students in the grade 10 students of La Salette of Roxas College, through the registrar office.
The information will be kept in outmost confidentiality and this personal information will not be
mentioned in the Research Paper unless being permitted to.
