Historical Perspective

The University of Perpetual Help Rizal


JONELTA Foundation School of Medicine
was established in June 1996, as an
extension school of the Perpetual Help
School of Laguna for Medicine and Allied
Health Science and was thus known as
the Perpetual Help College of Medicine-
Rizal (Extension School), with Dr. Aretas P.
Singson- Alday as its Founding Dean. On
April 20, 1997 by virtue of Resolution No.
R95-57 series of 1997 of the Commission
on Higher Education, the Perpetual Help
System Campus in Laguna and Rizal
became the University of Perpetual Help
Rizal College of Medicine.

Through the e!orts of its President,


Dr./BGen Antonio L. Tamayo and wife,
Daisy and its founding Dean, Aretas P.
Singson-Alday, MD the Medical institution
was granted autonomous status by the
Commission on Higher Education on
November 7, 1997 and became the
University of Perpetual Help Rizal
JONELTA Foundation School of Medicine.
Paying tribute to the patriarch and
matriarch, Dr. Jose and Nena Laperal
Tamayo whose vision for education
became a legacy and the Perpetual Help
System a living testimony to its ideals.

1. The scholarship is
open to all students,
whether new or
transferees, who are
graduates of any
bachelor degree with
academic honors.
2. The applicant must be
an NMAT exam passer
with the score of not
less than 50% as
determined by the
Board of Medical
Education.
3. The applicant must
undergo and pass the
Dean’s interview.
4. Qualified applicants
shall be entitled to the

5. The applicant must


enroll at least the
regular load for the
semester. Once
enrolled, it is understood
that the student must
not obtain a grade lower
than 80% in any subject
and the GWA per
semester must be at
least 85% to continue
enjoying the privilege.

8. NBI/Barangay/Police
Clearance
9. Eight (8) passport-
size, recent photographs
10. Certificate of
Honorable Dismissal for
transferees
11. Medical & Physical
Examination
12. Certificate of
Immunization, e.g.
Hepatitis, Tetanus,
Varicella, Tdap
13. For foreign students:
HbsAg, AntiHbs,
AntiHCV, HIV 1&2
Antibody Test and
Sputum AFB Smear
14. Recommendation
from the Committee on
Admission

+ NMAT score cut-o! is


50 percentile (based
on CMO No. 18 Series
of 2016 Section 17.3
and 17.5). Students
with an NMAT score
below 50 percentile
are given conditional
admission into the
program; hence, a
failing mark in any
subject at any year
level at the end of
each academic year
will mean automatic
dismissal.
