AFFIDAVIT OF LOSS

I, SHAIRA MAE A. CAINGHOG, of legal age, Filipino and a

resident of Block 4, Lot 35, Purok 14, Horizon Subd., Brgy. San Vicente,
Butuan City, Agusan del Norte, being duly sworn to according to law
hereby depose and say:

1. That I am the Finance Staff of “MACKGAB CORPORATION”

located at Block 4, Lot 35, Purok 14, Horizon, San Vicente,
Butuan City, Agusan del Norte;

2. That the original copy of CERTIFICATE OF INCORPORATION

issued to MACKGAB CORPORATION under Company
Registration No. 2022050051906-12, from Securities and
Exchange Commission, was lost;

3. Despite diligent search and efforts to locate the said certificate, I

could not find the same, such that I now believe that said is
now lost beyond recovery and remain lost to date;

4. As such, I am executing this Affidavit of Loss to attest to the

truth of the foregoing and for whatever legal purpose it may
serve.
