Dear Madam Krisel,

I am writing to inform you that I must resign from my position at the Local Government Unit of Tabaco –
IT Unit, effective immediately. Unfortunately, due to health issues, I am unable to continue working.

I have greatly enjoyed my time working for you and for the City Government of Tabaco, and I appreciate
the opportunities that I have been given to learn and grow within the organization. However, my health
issues have made it impossible for me to continue in my current role.

I would like to thank you for the support and understanding that you have shown me during my time
here. I am grateful for the experience and knowledge that I have gained through my work, and I will
always look back on my time at the company with fondness.

Please let me know if there is anything further that I can do to assist with the transition process to
ensure a smooth handover of my duties and responsibilities.
