1. About the project

The Government of Gujarat and Government of India has been implementing various
developmental schemes to improve the living standard of the people of the country. If
these schemes are implemented properly, all round development of the people will take
place which can lead to improve their leaving standard. But it is a reality that, due to lack of
knowledge about the various schemes, the benefit of the schemes is not reaching to the
real beneficiaries.

Keeping this in view the “SWAS” (SOCIAL WELFARE AWARENESS SCHEME) project was
started by The Young Citizen of India Charitable Trust under the social responsibility of
Suzuki Motor Gujarat Pvt. Ltd. in the nine project villages namely Sitapur, Hansalpur,
Jalisana, Mera, Naviyani, Finchadi, Shankhalpur and Becharaji-Bechar.

The purpose of the “SWAS” project is to create awareness about various government
schemes, provide adequate information and guidance to the concern beneficiaries of the
project area so that, they can avail the benefits of the respective schemes.

2. Objectives of the project: -

a. To enhance awareness about various health, nutrition, education and

employment related schemes.
b. To facilitate coordination and cooperation among community and concern
departments to avail the benefits of the various schemes.
c. To enhance networking among elected members, community and concern
officers to ensure the benefits of the various schemes to community.

3. Activities of the project: -

1. Activities of govt.meeting:-
Mamlatdar shree block Becharaji, District Mehsana-

Welfare officer meeting in sitapur Village:

On 05/09/2022 went to mandal taluka to collect widow and old age orders, its
operator Hasmukh Bhai gave us 17 old age orders from Sitapur which we got.
A meeting was held with Mamlatdar Saheb and Nyaya Mamlatdar Saheb of Mandal Taluka in
which the project work was discussed. There was also a discussion for the order of the
beneficiaries of Hasalpur and Sitapur villages.
On 24/11/2022 went to Patdi Taluka Panchayat and Taluka Vikas Adhikari office to give
forwarding letter. Also went to give form for widow and old pension of Naviani and Mera
villages.

MANDAL MAMLATDAR OFFICE:-

Talked to Mamlatdar Saheb shree of Mandal Taluka to take the order of the beneficiaries of
hasalpur village who asked us to give order on another date. And asked to get the ration cards of
some beneficiaries of Sitapur village Antodayacard, so that those beneficiaries get more benifit.

MANDAL MAMLATDAR OFFICE:-

When the first meeting was held with the Mamlatdar sir and Deputy Mamlatdar of Mandal taluka
for the widow pension order of Hasalpur village, there was talk of the widow's order. In this
context, Mandal went to take orders and gave orders for widow assistance to him as per the list.
And talked about the work in the village. There was talk of helping the beneficiaries of the
village to get more benefits.

2. Activities of Office meeting: -

Staff meeting-

The President and Founder have two meetings every month with the staff of SWAS project. In
that regard, a meeting was held on 16/08/2022 and 28/08/2022 in which the issues raised in the
previous meeting were discussed, the work done as per the Gant chart and the future planning for
completing the remaining work was discussed.

MEHSANA OFFICE MEETING:-

Staff meeting-

The President and Founder have two meetings every month with the staff of SWAS project. In
that regard, a meeting was held on 22/09/2022 in which the issues raised in the previous meeting
were discussed, the work done as per the Giant chart,Account file and the future planning for
completing the remaining work was discussed.

03/12/2022 Mehsana Office Meeting:-

A project review meeting was held at Mehsana office on 03/12/2022, in which the ongoing work
of the project was discussed and further work was planned, and the beneficiaries who have
benefited from the breath project There was a discussion about taking the Panchayat letter from
the Sarpanch of the Panchayat.

3. Activities of faliya meeting: -

Initially project was launched in selected five villages. A round table meetings were organized in
these villages. During these meetings dialogued were held among officials of the Suzuki Motors
company, representatives of the young citizen, Sarpanch/ elected members and local leaders.
During this process they were oriented about the project, activities and their role in it. Later one
faliya meeting in becharaji village.

Village level meeting and awareness activity-

Under the SWAS project, according to the gant chart, faliya meeting, local leader meeting and
women meeting were conducted in which people were given information about the welfare
schemes of the government and its necessary documents. In which meetings were mainly held at
Shankhalpur, Finchadi, Becharaji, Hansalpur.

Faliya meeting Becharaji 02/09/2022:-

On 02/09/2022, a board meeting was held at Bajania Vas in Becharaji village. About 22 women
were present. They were given information related to health and given information to take out
Ayushman card, and also gave information about how much rupees benefits.

Date 28/11/2022 faliya meeting Shankhalpur:-

A faliya meeting was held in Shankhalpur village, in which 22 women and men of faliya were
present. In the meeting, the beneficiaries were informed about Ayushman card and shram card.
Information was also given about the proof required for the card.
Date 29/11/2022 faliya meeting Naviyani:-

faliya meeting was held in Naviyani village, in which 17 women of faliya were present.
Beneficiaries were given information about Ayushman Card and Shram Card. And information
was given about the benefits of Ayushman Card.

Mera Faliya Meeting: -

A board meeting was held at Thakor Vas in Mera village, in which about 30 beneficiaries
come.Faliya meeting gave information to the beneficiaries in the meeting about the benefits of
widow and old age pension.
FALIYA MEETING BECHARAJI:-

On date 27/01/2023, a meeting was held at Becharaji in which 20 to 25 beneficiaries were

present. In the meeting, those who were asked to amend the Aadhaar card were asked to add
their name and date of birth and mobile number, so that they can get the Ayushman card easily.

SHANKHALPUR FALIAYA MEETING:- 31/01/2023

A faliya meeting was held in Shankhalpur village in which 25 to 30 beneficiaries were present
who were informed about the difficulties faced in Aadhaar card such as linking Aadhaar card
with mobile number and issuing new Aadhaar card to zero to five year old children and name
and birth inside Aadhaar card. He talked about correcting errors like date, asked those whose
name is not in the ration card to add their name inside the ration card and asked those who do not
have Ayushman card to produce the document.
03/02/2023 FALIA MEETING HANSALPUR:-

Hansalpur village jalasaniya Vas held a faliya meeting in which 15 to 20 number of beneficiaries
were present. In which the widows who were women were informed about what to do and what
documents would be required to ensure that the widow pension was not stopped. He explained to
those who have not linked the number to the Aadhaar card and asked the widowed women to do
the KYC of the Aadhaar card with their bank so that their money is not stopped. At the same
time, information was also given about ayushaman card. So that the hospital can help them in the
time of illness.
06/02/2023 HASALPUR FALIYA MEETING :-

A meeting was held at Bharwad Vas in Hasalpur village in which twelve women were present
who were informed about widow and old pension out of which two women benefit from the
“swas” project. In the meeting, he gave information about Ayushman Card and explained to
them about its benefits. The beneficiaries were asked to link the number to their Aadhaar card so
that it is easily for them to get an Ayushman card.

14/02/2023 Finchadi faliya meeting

A faliya meeting at Darbar Vas in Finchadi village and informed the beneficiaries of the faliya
meeting about the benefits of Ayushman Card and Shram Card and other government welfare
schemes such as widow and old age pension. Beneficiaries who have not filled the forms of
widow and elderly old pension were asked to fill the forms of that scheme.
BECHARAJI FALIYA MEETING :- 04/03/2023

He held a meeting at Indiranagar in Becharaji village in which 15 to 20 women were present who
were informed about schemes like old and widow PENSION and also about the benefits of
Ayushman and Shram card and asked the beneficiaries of widow assistance and old age
assistance to go to the bank and do KYC so that their pension is not stopped. He explained.
BECHAR FALIYA MEETING:- 14/03/2023

A meeting was held in Bechara in which 14 to 15 beneficiaries were present who were informed
about the labor card that they were working as retail laborers in the retail labor vegetable
business market and the workers working in the companies were asked to remove the labor card
or how much benefit it gets, as well as information about the benefits of Ayushman card and the
mobile card in the Aadhar card. He was also asked to link the number.
Faliya meeting naviyani:-20-03-2023

A meeting was held at Naviyani in which the beneficiaries were informed about the labor card,
as well as the laborers who do retail labor, drive a lorry in the market, do retail business, they
were asked to remove the labor card and also informed about its benefits, in addition to the pan
card and Aadhaar card that they have been asked to link with the current PAN card and Aadhaar
card so that they do not suffer much after March 31 as well as information about the Ayushman
card.
4. Activities of Local leader meeting:-
Initially project was launched in selected five villages. A round table meetings were organized in

Date 29/11/2022 meeting gram panchayat mera:-

Talked to Sarpanch Saheb, Talati Saheb and other members in Gram Panchayat in mera village
to collect the documents of the beneficiaries of my village as soon as possible.
Naviyani Gram Panchayat Leader Meeting:-

A leader meeting was held in Naviani Gram Panchayat in which Sarpanch Saheb, Talati Saheb
and members of Gram Panchayat were present. In the meeting, the beneficiaries who are left
without benefits in Naviani village and who do not know about the scheme were asked to convey
information through Gram Panchayat members. Asked to get the help of the members of the
Gram Panchayat.

On date 13/07/2022, Finchdi Gram Panchayat Sarpanchshree and Talatishree AD Chaudhary met
and started work in Finchdi village. A letter was given regarding that. And discussed with
Talatishree about the welfare schemes of Sarkarsree And told him about this project, and how
our organization is working in the village and asked him to cooperate during this operation
5. Activities Women’s meeting :-
Village meeting finchadi:- WOMEN’S MEETING

A women's meeting was organized at finchadi village on 07-07-2022. Village leaders and village
women participated in this meeting. In this meeting women were given information about the
welfare schemes of the government. How are these schemes implemented, Which documents are
required under this plan Information was given about all of them. What supporting evidence is
required in the plans at this meeting? Who is this scheme applicable to All those details were
discussed in detail above. In this meeting, along with those schemes, forms of schemes like old
age pension scheme and widow pension scheme were also distributed. Coordinator Khant
Pintubhai and Mobilizer Gosai Jaswantbhai, working in village breath project, took all the
responsibility to ensure that the benefits of many schemes run by the government reach the
people and every needy people get the benefit of this scheme.

SHANKHALPUR:-

On 31/10/2022 Bajania Vas mahila meeting in Shankhalpur village in which women and men
were present in the number of 20 to 25. They were informed about the scheme, and informed
about the Ayushman card and how many rupees it will benefit the hospital.

13/02/2023 JALISANA MAHILA MEETING:-

A women's meeting was held at Thakorvas in Jalisana village in which 19 women were present
who were informed about the welfare schemes of the government such as widow and old pansion
Ayushman Card and Shram Card and asked the beneficiaries living in and around their homes to
inform about these schemes and how much benefits are available from these schemes.
SITAPUR WOMENS MEETING :- 27-02-2023

A women's meeting was held in Sitapur village in which 30 to 32 women were present who were
informed about schemes like old age assistance and widow assistance and also informed about
the benefits of Ayushman and Shram card, the beneficiaries who do not have full name and date
of birth in the Aadhaar card were informed about the amendment and the beneficiaries of widow
assistance and old age assistance were asked to go to the bank and do KYC so that their Pension
will not stop coming.
Finchadi womens meeting:- 21-03-2023

A women's meeting was held in Fichdi village in which 18 to 20 women were present who were
informed about the labor card. In which the beneficiaries were asked to get the labor card of the
sisters and brothers who are engaged in retail labor, animal husbandry, farm labor so that they
can get necessary assistance from the government. He also informed the women about widow
and old pension. So that the beneficiaries living in their vicinity get the benefit of this scheme,
they also gave information about the Ayushman card so that the beneficiaries who are yet to
withdraw the Ayushman card should be removed as soon as possible so that they can use that
card in case of any illness.
6. Other Activities: -

Pre-testing of the pamphlet to be prepared for Information Education and Communication

under Shwas Project, which was revised and then will be sent for approval.

Approval of Final Budget and Establishment of new project office

Suzuki Motor Gujarat Pvt. Ltd. sanctioned the budget which was kept by Young Citizen of India
Charitable Trust on 20th June, 2022. After the appoval of the budget new poject office (B-111
Seth prahlad bazar, opp. Umiya vadi, Shankhalpur road, Bechraji) was established on 1 st July,
2022 with required accessories like computer, printer, scanner and furniture.

Mrs. Sonal Mehta (President) and Dr. Amar Vyas (Founder) went into field work to check the
ongoing activities of the SWAS project and done meeting with sarpanch, local leaders and
village people, after this, meeting was done with Mr. Kalpesh Mehta (Senior Manager, CSR) in
which there was discussion of evaluation of field work, meet new recruited staff, talked about
new project office and about futue plans.

As the Ayushman Card worked well across Gujarat and the beneficiaries got more benefits,
Gujarat received the Ayushman Excellence Award across the country.

After completing the Base line survey in September, the beneficiaries were left with field work
in the villages, they had the task of distributing forms, and also gave them information about the
schemes, and also gave information about the necessary documents to the beneficiaries.
There was a medical camp in Hasalpur village in which breath project also participated, in which
beneficiaries from neighboring villages came, those who did not get benefits like widow aid and
old age aid were asked to take this benefit and those who did not fill the forms were given new
forms.
Ayushaman card work: -

The work of Ayushman card was done in fichadi, Mera and Naviani villages. We go and issue
Ayushman card to the beneficiaries who cannot go out and get this card. We give those
documents to the beneficiaries who are missing some documents. And also we give information
about other schemes.

Field work Mera and Shankhalpur: -

In Mera village Ayushman Card worked by going home to beneficiaries who were not able to go
and draw themselves, including young children and the elderly. Meanwhile in Mera village there
were applications from old people and widows and other beneficiaries were being informed
about the schemes of the government.
In Shankhalpur the applications of the beneficiaries given above came to Shankhalpur Panchayat
which we received. And which was to get other beneficiaries to prepare their documents soon.

7. Activities of workshop: -
Village Level Workshop: -

(A) Several developmental schemes have been implemented for the people of the Government
under the Swas Project. A program was organized with the beneficiaries through the help of
Suzuki Motors Gujarat Pvt Ltd.Swas Project Antargartha Young Citizen of India Charitable
Trust, with the aim that the villagers get the information about the entire schemes inspired by the
government and the people from the outskirts of the village can benefit from it.

(B) In the program, out of total 27 beneficiaries out of total 27 beneficiaries of the villagers who
have benefited from the welfare schemes of the government under the breath project, MD Sir of
Suzuki Motors Gujarat Pvt. The benefit papers of the incentive scheme were given by the hands
of Sarpanchshree, Talatishree, village leaders, beneficiaries and villagers were present in the
programme.

(C) Managing the program Dr. Amar Vyas said that during the time of monarchy in the country,
there was a system of Mahajans, the best help the needy people and do public interest works like
schools, hospitals, etc. Presently Gujarat state is a welfare state which has about 25 different
departments which

have more than 300 welfare departments. Schemes are implemented in rural and urban areas
Lack of documentation to convey the benefits of the scheme to the man of the village or due to
some unexplained reasons some beneficiaries are not getting the benefit which is the
implementation of the project by young citizens under the breath project started to fill the
missing link with the support and guidance of the officials of Suzuki Motor Gujarat Pvt Ltd. has
been done.

WORKSHOP PROGRAM IN MERA VILLAGE:-

On 27/09/2022 a workshop meeting was held in Mera village in which Ayushman Card and
Shram Card were distributed. Dr. Amar Vyas Sir discussed the welfare schemes of the
government, as well as destitute, old age, and widow pension and crisis relief Plans were
discussed. Also, member was appointed vice-chancellor in the village. A committee was formed
in Mera village under the “SWAS PROJECT”, in which the members of the village have been
appointed.
10/12/2022 Finchadi Workshop:-

Finchdi village organized a workshop program in which Dr. Amar Vyas, founder of the Young
Citizen Institute and Sonalben Mehta, president of the organization, former village sarpanch
Saheb and village leaders and village beneficiaries were present in which Ayushman cards and
Shram cards were distributed which 62 Ayushman Card and 16 Shram Card beneficiaries were
given the card by the guests present.
Mehta Workshop distribute program Hansalpur: -21/02/2023

The workshop was organized in Hasalpur village to distribute orders for the benefits received by
them to the beneficiaries of widow and old pension. workshop programme was attended by 60
beneficiaries including beneficiaries of old and widow pension.

Mr. Kalpeshbhai and Mukeshbhai, Head of Suzuki Gujarat Pvt. Ltd. and Mr. Mukeshbhai, Head
of Suzuki Gujarat Pvt. Ltd. and Mr. Amar Vyas, Founder of Young Citizen Organization, Sonal,
President of the Organization, Pintubhai, Nitinbhai and Jaswantbhai, activists of The Young
Citizen Organization, were present in the workshop program. The sarpanch of the village, the
deputy sarpanch, the member of the village and the members of other congregations were present
and the program was organized. The orders were distributed to the beneficiaries at the hands of
guests and members.
Sitapur workshop 18-03-2023:-

Workshop program was organized on 18-3-2023 in Sitapur village. Dr. Amar Vyas, Founder of
Young Citizen Organization, Sonal Mehta, President of the Organization and Kalpeshbhai and
Mukeshbhai Saheb From Suzuki Motors Gujarat attended the program. Six widow assistance as
well as 22 old pension orders were distributed in the programme.
 4. Future plan
I. Prepare a pamphlet and sticker.
II. Organize meetings and disseminate pamphlets.
III. Meeting government officials and local Leaders.
IV. Continue process of filling up of new forms/ applications for availing benefits of
the schemes.

To submit the forms/ applications of the villages to the concern government departments.
