    Childhood / Adult Diseases- As stated by the client, before she was diagnosed
    with Acute Lymphocytic Leukemia, she only experienced fever, cold, and coughs
    whenever she was playing in the rain.
    B. Injuries / Accidents-The client fell down from a 15 steps of stairs when she was
    16 years old.
    C. Hospitalization: As stated by the client’s mother, the client was hospitalized
    when she started to have a heavy period which lasted for 8 months causing her
    to have a blood transfusion.
    D. Operation- As stated by the client, she did not undergo any operation before.
    E. Allergies- The client does not have any allergies with foods. However, the client
    is known to have allergic reactions to Amphotericin-B.
    F. Medication: Oral Meds: Spironolactone 50 mg tab q12, Bilastine 20 mg tab q12,
    Allopurinol 150 mg tab q8, Calcium Carbonate 1-tab q8
    G. Immunization: The client is fully vaccinated against COVID-19 Virus by the
    Pfizer vaccine. Her first dose was on August 19, 2021 while her 2 dose was on
