COHSASA REQUIREMENTS FOR CLINICIANS’ ATTENTION
1. Full involvement in COHSASA processes (SOP development, implementation of standards, SOPs,
guidelines, 5-star customer experience, etc.). Pay attention to the hospital documents on LH manuals. 2. Completeness of patient records 3. Documented hand over 4. Triage, Initial and ongoing assessment of patients 5. Discharge planning at initial assessment 6. Discharge process. Discharging patients through the relevant members of the multidisciplinary team (e.g. counselors, social workers, physiotherapists, nutritionist, chaplaincy, etc. depending on the patient’s needs), to ensure continuity of care, especially those that require rehabilitation. 7. Informed consent 8. Owning one’s work, including verbal prescriptions (by way of signature or initials). Note: include time when these prescriptions are made. 9. Health education / Educating patients about their condition (with documentary evidence) 10. Involving patients or their care takers in care / Management 11.
