MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT entered into and between: 

Quezon Provincial Plans and Programs Unit, a government agency established and
existing under the laws of the Republic of the Philippines, with address at Camp
BGen Guillermo P Nakar, Barangay Gulang Gulang Lucena City, represented
herein, by PMAJ ALEJANDRO D. ONQUIT, Acting Chief, Quezon Provincial Plans
and Programs , herein referred to as “Host Training Establishment or HTE”.

-and-

DALUBHASAAN NG LUNGSOD NG LUCENA, a duly accredited educational


institution with business address at City College Main Bldg. Isabang, Lucena City,
herein represented by DR. MARIA CHARMAINE V. LAGUSTAN,
President, hereinafter referred to as the “DLL”.

WITNESSETH THAT:

WHEREAS, DLL instituted an on-site internship program as part of the


curriculum for its Bachelor of Science in Public Administration students in order to
provide them with adequate experience in actual industry work operations and to
augment their formal education;

WHEREAS, the HTE, in its desire to enhance the training and development of


more and more professionals, has agreed to provide support to the students of DLL
in the form of training on the operations and latest developments in its industry; and

WHEREAS, DLL and the HTE executed a Memorandum of Agreement that


covers the internship program’s objectives and principles pursuant to their
established Internship Plan for the Students.

WHEREAS, the Student accepts this Agreement and the respective


Internship Plan with all its terms and conditions as contained herein.

NOW, THEREFORE, for and in consideration of the foregoing premises, the


Parties hereby agree as follows:

 
1. COMMON PROVISIONS

1.1 The HTE and DLL, hereby bind themselves to undertake this


Memorandum of Agreement as a bond outline of their intentions to
cooperate and promote the academe- industry partnership;

1.2  Students shall abide by the administrative policies, rules, and standards of
the HTE and DLL as provided by the DLL Student Handbook;

1.3  This Agreement shall not, in any way, constitute an employee-employer


relationship between the HTE with the DLL On-The-Job Trainees;

1.4 In the event that the student materially violates any agency policy, the HTE
shall have the right to immediately, with due process, terminate the
Internship Plan in relation to such Student by immediately serving a written
notice to DLL;

1.5 Both PARTIES agree the following practicum requirements:

a. The student shall undergo 200 hours of on-site practicum and must
secure the recommendation from the Dean or Practicum Coordinator
as official endorsement to the HTE.

b. The student who is found to misbehave and/ or act in defiance to the


existing standards, rules and regulations shall voluntarily withdraw.

c. The student shall be personally responsible for any and all liabilities
arising from negligence in the performance of his/her duties and
functions while under training.

d. Practicum consultation/ online video conferencing with the HTE is done


subject to their approval at reasonable time convenience to the first
and second parties.

e. Failure of compliance with the requirements and guidelines in the given


time, the practicum will be deferred of forfeited.

f. This agreement shall become effective upon signature of both parties


and implementation will begin immediately and shall continue to be
valid hereafter until written notice is given by either party fifteen (15)
days prior to the date of intended termination or upon compliance of
the student-trainee’s 200 hours on the job training or whichever comes
first.

g. This agreement will be effective within two consecutive academic


years.
1.6 Should any provision of this agreement or part thereof is render void,
illegal, or unenforceable by any law to which it is subject of, it shall be
rendered void, illegal, or unenforceable only to that extent and not further.
The invalidity or unenforceable of any provision of this agreement shall not
affect or impair other provisions that are valid, binding and enforceable.

2. That DLL, represented by the Dean/ Department Head/ Program


Coordinator/ Practicum Coordinator, in relation to the on-site practicum
shall undertake the following responsibilities

2.1 DLL must submit the names of the students (prospective


interns) to Quezon Police Provincial Office at least a week
before the desired commencement date of the internship;

2.2 DLL must secure that the interns are fully vaccinated and
with active Philhealth membership;

2.3 DLL shall submit to HTE the evaluation form to be used in


the performance evaluation of the interns;

2.4 Make periodic checks of students, either through random


actual location visit or telephone calls to monitor his/her
progress;

2.5 Perform any other related task and activities deemed to be


necessary for the successful conduct of the practicum
program.

2.6 DLL shall NOT, in any way, be responsible if any of the


interns become infected with COVID-19.

3. That the HTE agrees and promises to provide the Internship Program
and commits to perform, through the authorized representatives, the
following responsibilities:

2.7 Provide the student with a maximum of 200 hours of actual


job experienced and training (knowledge, skills and right
attitudes) which are relevant to their respective courses and
will match the current industry requirements;

2.8 Ascertain the safety and protection of the students while


inside the agency’s safety and by refraining from giving them
hazardous tasks;

2.9 Give orientation about the work values, behavior, and


discipline to ensure smooth cooperation with the interns.
2.10 Supervise the interns to ensure that the existing standard,
rules and regulations are properly followed.

2.11 Ensure that the interns are posed to the following areas
of concentration:

i. Clerical works
ii. Administrative
iii. Other Office Works

2.12 Evaluate the intern’s performance and provide the


school’s representative with an honest and objective
evaluation of the students OJT performance prior to or upon
the completion of 200 hours;

2.13 Accommodate the school’s representative during random


checking of the students’ progress (either during telephone
calls or actual office visit) and provide the performer with
essential and honest feedback about the students;

2.14 HTE shall NOT, in any way, be responsible if any of the


interns become infected with COVID-19.

4. That the STUDENT/ PRACTICUMER shall undertake the following


responsibilities:

4.1 As part of the curriculum of the BS Public Administration in


Dalubhasaan ng Lungsod ng Lucena, the students are required
to undergo 200 hours of on-site practicum;

4.2 The student is required to enroll the Practicum subject for 6


units to be officially registered.

4.3 Comply with the requirements and guidelines of the


Internship Program;

4.4 The student is required to have oriented about the work


values, behavior and discipline to ensure smooth cooperation
while on OJT.

4.5 Attend the scheduled consultation meetings with the


Practicum Coordinators;
4.6 Observe and follow all the HTE’s rules, policies, code of
conduct, code of discipline, code of ethics safety procedures,
and the likes for the effective, safe and successful completion of
the program;

4.7 Regularly report to the HTE for the practicum work and notify
the immediate superior or any key personnel in case of inability
to do so for any reason whatsoever;

4.8 To exercise care, due diligence and safety in using all HTE’s
resources and to return any agency property issued (to the
students) in good condition upon the completion of the OJT;

4.9 Perform all reasonably OJT/ PRACTICUM related tasks,


duties and responsibilities assigned by the immediate superior;

4.10 At the end of training course, he/she is required to submit


the following items to the professor in-charge of Practicum:

a. Certificate of Completion
b. Weekly Reports
c. Evaluation Form
d. Documentation (daily time record, brochures, photos
and the like)

IN WITNESS WHEREOF, we signed this Memorandum of Agreement, this
