On basis of relevant requirements of the Manufacture Licensing, applicant shall have
related main production equipment, including manufacture equipment and heat treatment
furnace whose capacity is suitable for production of the products applying for
manufacture license. The quantity and capacity of them filled in this form shall meet the
Requirements for Boiler and Pressure Vessel Manufacture Licensing. Additional paper
can be attached if necessary. (Please refer to the Annex 2)

According to the relevant stipulations of Manufacture Licensing, applicant should have
corresponding main inspection and testing instrument including physical-chemical testing
devices. The capacity and quantity of them filled in this form shall meet the
Requirements for Boiler and Pressure Vessel Manufacture Licensing. Additional paper

13.1Means for submitting the application documents
The applicant shall submit the Application Form for Special Equipment Manufacture
Licensing in quadruple accompanying other documents required to the Special
Equipment Licensing Office (hereinafter called SELO) of AQSIQ. After acceptance by
the AQSIQ, the acceptance organ, application manufacturer, the survey and assessment
organ shall keep the application form respectively. In addition to submit application form,
the applicant is asked to submit an electric copy of file (by e-mail, flip disk or compact
disc).
13.2 Means of acquirement of licensing application documents
Applicant may download the application form and other relevant documents for licensing
application as follows:

The application form shall be printed in form of A4 size paper with over 70g weight.

Appendix 3 Resource Requirements for Boiler Manufacture Licensing

1 General Provision

1.1 The Requirements for Boiler Manufacture Licensing are specially formulated in
accordance with relevant requirements in the Supervision Administration Regulation for
Manufacture of Boiler and Pressure Vessel (hereinafter referred to as the "Administration
Regulation").

1.2 The Requirements apply to all boiler product manufacturers, which specifies in
“Administration Regulation” (hereinafter referred to as manufacturer).

1.3 The Requirements are comprised of the resources requirements for boiler
manufacture licensing, quality management system requirements and boiler safety quality
requirements. The resource requirements include basic requirements and specific
requirements. The basic requirements are the general requirements for producing all
kinds of boiler products and the specific requirements are the specific requirements for
producing relevant level of boilers , all of which should be met by the manufacturers
simultaneously.

1.4 Manufacturer shall establish, and continuously and effectively implement a sound
quality management system suitable for its boiler products. In order to verify the control
capability of its quality management system for boiler products, the manufacturer shall
have enough practice in continuous producing boiler products.

1.5 The non-destructive examination (NDE), heat treatment, and physical and chemical
property examination can be performed either by manufacturer itself or by subcontractors
with relevant qualification or competence, when the manufacturer signing consignment
agreement with them. The consignment agreement has to put forward to the license-issue
department on file. The consignee shall issue the related report of the consigned work to
the manufacturer and the manufacturer shall be responsible for the quality control of the
consigned work, which should be included in the quality management system for boiler

of the manufacturer. The requirements of the specific requirements for resource shall not

be consigned.

1.6 Manufacturer shall have a capability of producing main body (shell and/or head) of

products to the subcontractors.

2.1 The applicants for boiler manufacture licensing must have independent legal status or
have registered in local governmental departments where the manufacturer is located.

2.2 The boiler manufacturer must have suitable technical ability for boiler production and

2.2.1 The manufacturers should have enough technical staff for various technical
processes including boiler manufacture, machining, NDE, welding, material examination

and quality management. The technical staff of the manufacturer with license of level A
and level B have to be no less than 10 per cent of all its employees, and those of level C
and level D have to be no less than 5 per cent and not less than 5 persons. Enough
professional welding and boiler manufacture technical staff must be employed by boiler

manufacture.
2.2.2 The manufacturer shall have relevant personnel responsible for the stages in boiler
manufacture including design, technology, material, cold working, hot working,
machining, forming, welding, NDE, heat treatment, pressure test, product inspection and
testing, standardization, metrology and quality management, etc.
2.2.3 Qualified NDE personnel and NDE items, qualified welders and welding operators,
and welding items shall meet the requirements of the production of the products produced

2.3 The requirements for workplace and technical facilities:


2.3.1 The area and height of workshop should meet the requirements for manufacture of
boilers with the applied licensing level. The manufacturing process should be rationally
arranged. The welding of pressure parts of boilers shall be performed in workshop.
2.3.2 Pipes and semi-finished products should be kept at the place under certain
protection.
2.3.3 Warehouse for keeping welding consumables that require control of temperature
and humidity during storage must be equipped with equipment to ensure the required
temperature and humidity. Drying and temperature-maintaining oven to make these
welding consumables ready for use is also required.
2.3.4 There should be radiographic testing site that can meet both product requirements
and protection requirements. There should be basic conditions to ensure quality of
developed negative film and the storage of the radiographs.
2.3.5 NDE facilities that fit for the manufacture of the products are required except NDE
is performed by subcontractors.
2.3.6 Fixtures are required to meet demand of boiler product manufacture.

2.3.7 The Lifting capacity at main workshops should meet the manufacturing demand of

boiler products.

2.3.8The drilling and pipe-bending machines should meet the manufacturing demand of

boiler products.
2.3.9 Welding machines that fit for boiler product manufacture are required.

2.3.10 Examination Platform and tools, and hydrostatic pressure testing facilities suitable
for production are also required.

2.3.11 Mechanical property testing facilities, physical and chemical property examination
devices or a consignment relationship of the above mentioned testing and examination
with competent subcontractors are required.
2.3.12 The applicants should have the machining tools, including cutting machines,
presses and bevel machines for both plates and pipes to meet the demand for the
manufacture of boilers of applied licensing level. Compact boiler manufacturer must have

2.4. The resources requirements for the manufacture licensing of organic fluid heater and

the resources requirements for level A boiler parts may consult the relevant resource
requirements for the manufacture licensing of level C and level A boilers respectively.
Furthermore, the manufacturing equipment required should match the manufacturing
products.

3.1 Specific requirements for manufacture licensing of level A boiler
3.1.1 Requirements for technical resource:
(1). There should be chemical and physical property testing lab and metallographic
examination lab suitable for the manufacturing products
(2). There should be welding labs suitable for manufacturing products
(3). There should be R&D capacity for design new products and the capacity for
production of the new products.
(4). There should be enough staff for standardization, metrology and enough full-time
inspectors
(5). There should be NDE examiners that hold RT III and UT III certificates in addition
the examiners with level II certificates in RT, UT, MT, PT. Provided the NDE are
consigned outside, the above requirements for examiners holding RT II and UT II
certificates may be exempted.
(6). The number and granted item of qualified welder and welding operator should be
satisfying the requirements of production, it should be not less than 50 person-items in
general.
3.1.2 Requirements for manufacture equipment and fixture:
(1). The capacity of semi-automatic or automatic cutting machines should meet the
cutting thickness demand for manufacturing level A boilers.
(2). Welding equipment including automatic arc welding machine, gas shield arc welding
machine and manual arc welding machine, etc. should meet the demand of the
manufacturing products.
(3). Level-A manufacturers must have two of the following three significant facilities:

3.2 Specific requirements for manufacture licensing of level B boiler


3.2.1 Requirements for technical resource:
(1) There should be enough capacity for changing the requirements in drawing to actual
manufacturing technology.
(2) There should be enough full-time inspectors.
(3) The qualified RT II and UT II examiners should not less than 2 persons respectively.
The figures can be decreased to 1 person respectively provided the NDE are consigned
outside.
(4) The number and granted item of qualified welder and welding operator should be
satisfying the requirements of production and not less than 30 person-items in general.
3.2.2 Manufacture and inspection and testing equipment
(1). Press or the ability to coordinate quality warranty subcontractor is required to meet
the demand of products
(2) Plate bending machine is required to meet product demand (with bending capacity of
20-30mm thickness of steel plate in general)
(3) The maximum lifting capability in main workshop should satisfy the product demand
and normally be not less than 20 tons.
(4) Enough welding equipment including automatic submerged arc welding machine, gas
shield arc welding machine and manual arc welding machine, etc. are required to meet
the demand of products.
(5) Mechanical property testing machine and machine tools and instruments for preparing
impact testing specimen or the ability to coordinating all the quality warranty tests are

3.4 Specific requirements for manufacture licensing of level-D boiler


3.4.1 Requirements for technical resource:
(1) At least one qualified RT II NDT examiner is required..
(2) The number and granted item of qualified welder and welding operator should be
satisfying the requirements of production and not less than 10 person-items in general.
3.4.2 Manufacture and Inspection and Testing Equipment
(1) Equipment for cutting, welding, drilling, pipe bending, plate bending and machining
is needed to meet the product's demand.
(2) Forming and machining equipment are required to meet the demand of manufacturing
products.
(3) The maximum lifting capability in main workshop should satisfy the demand of
manufacturing product and normally be not less than 5 tons.
At least one radiographic detector in good condition is required to meet the demand of
the manufacturing products provided the manufacturer itself performs the
NDT.

4. Resource Requirements for Manufacture Licensing of level A Boiler Parts

The term “Parts of boiler A level” refer to the following pressure parts used on steam

boiler (including waste heat boiler) with the rated output steam pressure greater than

2.5Mpa,such as the boiler drum, membrane water-cooled wall, economizer, superheater,

reheter and header, etc. The manufacturer fabricating the aforementioned pressure parts
shall obtain corresponding manufacture license of “part of boiler A level”.

4.1 Basic Requirements

The basic requirement for level A boiler parts shall satisfy the stipulations of 2 in this
appendix.
4.2 Specific requirements
4.2.1 Technical resource
Technical resource shall comply with Article 11 of “Requirement for Boiler and

Pressure Vessels Manufacture Licensing”. For the boiler parts manufacturer who doesn’t
fabricate the boiler drum, RT and UT level Ⅲ examiner required by NDE allow to be
exempted.

4.2.2 Requirement for the tool and equipment
Manufacturer of boiler drum shall meet the requirement of Article 11 of
“Requirement for Boiler and Pressure Vessels Manufacture Licensing”. Among these
equipment, the subcontract of hydraulic (oil) pressure and heat treatment equipment is

permitted if the products quality is ensured under subcontracting. Membrane
water-cooled wall manufacturer shall have the especial equipment such as auto-welding
machine with multi head, flattening and panel bending machine, finishing equipment for
flat steel. For the boiler manufacturer fabricating economizer, superheater and reheater,
the equipment and tool which suitable for tube butting welding are required, such as
automatic hot wire TIG or TIG plus MIG welding machine etc. Simultaneity, the argon
arc welding machine, tube end forming machine, chamfer machines, groove preparation,
tube bender, cutting and rack for tube, etc. are also required. For the manufacturer of
boiler pressure piping and header of boiler, the necessary equipment, such as cutting,
bending, gas-shield welding, Arc welding, submerged arc welding of girth seam
(applicable only for the header fabrication), preheating and heat treatment equipment
(allow to subcontract),etc. shall be available.
4.2.3 Inspection and test equipment
Inspection and test equipment shall meet the requirements of Article 11 in
“Requirement for Boiler and Pressure Vessels Manufacture Licensing”, and the
manufacturer of alloy steel products shall have the equipment for the spectral inspection
4.3 Cast iron economizer
No manufacture license required to the foundry which produce the casting pieces
only, and which doesn’t fabricate and assemble the parts to the economizer.
4.4 Other boiler parts
The boiler parts, such as the internal furnace (including expansion joints of the
internal furnace), food ring, tube sheet, etc. may produced by the manufacture license
holder of pressure vessel head.

2.3 Quality assurance system staff for pressure vessel


Pressure vessel manufacturer should provide the following quality control system with
responsible persons having relevant professional knowledge, experience and suitable for
the manufacturing products.
(1). Responsible persons for design and technological process control system
(2). Responsible persons for material quality control system
(3). Responsible persons for welding quality control system
(4). Responsible persons for chemical and physical testing quality control system
(5). Responsible persons for heat treatment quality control system
(6). Responsible persons for NDT quality control system
(7). Responsible persons for pressure testing quality control system
(8). Responsible persons for final inspection and testing quality control system

2.4 Technical staff

The pressure vessel manufacturer should have all the professional technical staff that

2.5 Professional operator

2.5.1. Among all levels of pressure vessel manufacturers, the manufacturer of welded
pressure vessels should have qualified welders and welding operators to meet the demand
of products.
(1). Level A2 and level A3 and Level C licensing manufacturer should have at least 10

qualified welders and welding operators who have at least 4 granted items together.
(2). Level A1, level A5, level B2 and level B3 licensing manufacturer should have at
least 8 qualified welders and welding operators who have at least 3 together granted items
(except for non-welded vessels manufacturers).
(3). Level D licensing manufacturer should have at least 6 qualified welders and welding
operators who have at least 2 granted items together.
A. All levels of manufacture licensing pressure vessel manufacturers should have the
assemblers who can meet the requirements of pressure vessel production.
B. Among all levels of pressure vessel manufacturers, those who consign the NDE to
other enterprises should have high- and middle-level of responsible qualified NDE
examiners in accordance with the manufacture level. If the manufacturers themselves
perform the NDE, the qualified NDE examiners should meet the following requirements:
a. Level A1 licensing manufacturer should have at least one qualified high-level RT
(or UT, MT, PT) examiner who is responsible for NDE.(This stipulation has been
modified, please refer to Article 1 of Appendix 15 Revised Regulation for detailed
information)
b. Level C licensing manufacturer should have at least one qualified high-level RT
(or UT) examiner and two qualified middle-level RT and UT examiners
respectively.(This stipulation has been modified, please refer to Article 1 of Appendix 15

c. Level A2 and level A3 licensing manufacturer should have at least three qualified
RT and UT middle-level examiners respectively, and the responsible person for NDE

All levels of pressure vessel manufacturers should meet the following specific
requirements.
3.1 Specific requirements for level A pressure vessel manufacture licensing
3.1.1 Super-high pressure vessel manufacturer in level A1 licensing manufacturer should
have machining tools and inspection and testing devices suitable for producing
super-high pressure vessel and have at least 2 middle-grade or high-grade machinists.
The high-pressure vessel manufacturer should have heat treatment furnaces suitable for
production.(This stipulation has been modified, please refer to Article2 (1) of Appendix
15 Revised Regulation for detailed information)
3.1.2 Level A2 licensing manufacturer should have plate bending machine with rating
capacity no less than 30mm in plate thickness(This stipulation has been modified, please
refer to Article 2 (2) of Appendix 15 Revised Regulation for detailed information)and
lifting appurtenance with lifting capacity of no less than 20 ton(This stipulation has been
modified, please refer to Article 3 of Appendix 15 Revised Regulation for detailed
information). The cryogenic (isolated) vessel manufacturer should have equipment for
packing filling, drying, vacuuming and leak detector.
3.1.3 Spherical tank petal manufacturer among level A3 licensing manufacturer should
equip with press with capacity not less than 1200 tons and have experienced professional
operator in producing the petal.
3.1.4 Fiber wrapping vessel manufacturer among level A4 licensing manufacturer should
have automatic wrapping machine.

3.1.5 Level A5 licensing manufacturer should at least have 2 qualified electrician with

middle-grade or above and electric inspection and testing equipment.

3.2 Specific requirements for level B pressure vessel manufacture licensing

water jacket volumetric expansion.

policy is understood and implemented at all of its staff and should meet the following

interrelation among the different activities.

conforms to the specified requirements. The quality manual should include or make
reference to the quality system procedures and outline the structure of the
documentation used in the quality system.
2. The manufacturer should prepare documented procedures consistent with the
requirements and manufacturer’s stated quality policy, and effectively implement the
quality system and its documented procedures.
3. The forms required by quality manual should be documented with standard format.

welding repair, the requirement of re-examination of repaired weld and the process of
welding repair of flaws in base metal.
5). The manufacturer should establish the stipulation for keeping welding record of main
pressure parts.

AQSIQ. But, in this case, they must meet the requirement as stipulated from article 2 to 7
onwards simultaneously.

2 Requirements of steel for pressure components of boiler


Steel for pressure components of the boiler (including staying components) must be
killed-steels. The designations of the steel should be those listed in foreign boiler steel
standards, or of conventional boiler steels.

3 Requirements for boiler structure


The structure of boiler should be in complete accordance with above-mentioned
Chinese safety technical regulations for boilers.

4 Requirements product inspection


The results of visual inspection of boiler, mechanical property testing of welded joint,
metallograpy and fractography, hydrostatic pressure test, and the NDE items and
detective rate must satisfy the requirements of above-mentioned Chinese boiler codes and
regulations.

5 The requirements for safety appurtenances and metering devices

3) Calculation sheets for the required safe relieving capacity of pressure vessel, relieving
capacity of safety valves and/or the discharge area of bursting or a summary of above
calculation results.
4) Testifying papers of product quality including certificate of compliance of the product,
mill reports for main pressure parts, NDE reports, heat treatment reports, pressure test
reports and leak test reports, etc.,

4.1 The safety factor for allowable stress of materials (design safety factor) should be
determined in accordance with the following requirements:
In general, the safety factor for steel pressure vessel shall not be less than 3 if the
design is based on the tensile strength at room temperature. The factors should normally

not less than 1.6 for carbon steel and low alloy steel and not less than 1.5 for high alloy
steel if the design is based on the yield strength at ambient temperature. Otherwise, it
should subject to prior approval by the safety supervision administration of AQSIQ.
The safety factor for stress analyzing design should normally not be less than 2.6 if
the design is based on the tensile strength at ambient temperature and should normally
not be less than 1.5 if the design is based on the yield strength both at ambient and design

corresponding working pressure shall be noted on the design drawing.
4.7 Neither lap joint nor cruciform joint is permissible for connecting shell to shell, shell

to head and for connecting welds in welded heads.
4.8 One hand hole or two hand holes (for those vessels impossible to fit man-hole) should
be provided for the pressure vessels with inner diameter not less than 500mm (except for
those vessels, such as, jacket vessel, heat exchangers where the opening on the vessel are
not allowed).
4.9 The safety interlocking devices shall be provided for pressure vessels with
quick-actuating doors (closures).

5 Requirements for pressure vessel steels


5.1 The materials for pressure components of pressure vessel shall be used within the
applicable range of relevant standards.
5.2 The phosphor and sulfur contents of the carbon steel and low-alloy steel used for
main pressure components of welded pressure vessel shall not be greater than 0.030%
and 0.020% respectively.
5.3 The carbon contents in carbon steel and low-alloy steel used for main pressure
components of welded pressure vessel shall not be greater than 0.25%, and the carbon
equivalent Ceq should not be more than 0.45%. If the carbon and low-alloy steel, which
carbon contents are greater than 0.25%, are necessarily needed, the manufacturer shall
meet all the following requirements:
5.3.1 Obtaining agreement and acknowledge of the customer in advance,
5.3.2 Carbon equivalent Ceq of the material should not be greater than 0.45%,
5.3.3 Weldability test report and welding procedure qualification report of the material
should be provided, and report safety supervision administration of AQSIQ for review
and approval.

5.4 For those quenched and tempered low-alloy pressure vessel steel, if the specified

6 Requirements for manufacture
6.1 Cold-formed carbon steel and low-alloy steel convex concave heads should conduct

6.2 The following pressure vessels should conduct block post-welding heat treatment for
relieving residual stress:

6.4 For those pressure vessels, which is designed by fatigue analysis, the reinforcement of
welds in category A, and category B butt-joints shall be removed; all welded joints shall
have smooth transition.
6.5 All corrugate plate-and-shell type heat exchanger shall have dismountable and
cleanable structure.

7 Requirements for inspection


7.1 Each of the following pressure vessels should produce a longitudinal product welding
test plate:
7.1.1 The pressure vessels made of Cr-Mo low alloy steel or material with minimum
specified tensile strength more than 540 MPa in the relevant standard.
7.1.2 The pressure vessels with design temperature of less than -20 and requiring low
temperature impact test.
7.1.3 The pressure vessels requiring heat treatment to assure the mechanical properties of

7.2.12 Butt joints in inner cylinder of multi-layered pressure vessel and in each cylinder
of hot-shrink-fitted pressure vessels.
7.2.13 The pressure vessels which should be examined with 100% RT or UT in
accordance with design drawing.

7.3 Except for the requirements as specified in 2. Of this Article, it is permissible to apply
spot NDE for category A and B welded joints. The percentage for spot NDE shall not be
less than 20% of the length of each weld and not less than 250mm. But all of the
following welded joint should be non-destructive examined and the acceptable criterion
should be in accordance with the requirements of the pressure vessel.
7.3.1 All of the T-joints.
7.3.2 All welded joints within opening area (the area within the circle with diameter of
1.5 times of the opening diameter and having the same center of the circle as the opening).
7.3.3 The welded joint to be covered by reinforcement rings, saddles or pads.
7.3.4 Butt joint in welded head or welded tube plate.
7.3.5 All butt joints in nozzles with a nominal diameter greater than 250mm. The

percentage and acceptable criteria shall be the same as that of the welded joint in the

Appendix 8 Quality Requirements for Gas Cylinder Safety

1. The gas cylinders produced by gas cylinder manufacturer should meet the

1) Safety Supervision Regulation on Baric Oxygen Chamber for Medical Treatment
2) Safety Supervision Regulation for Gas Cylinders
3) Safety Supervision Regulation for Dissolved Acetylene Cylinder

4) Safety Supervision Regulation on Liquefied Gas Containers for Tankers

2. All gas cylinders must be designed and manufactured in accordance with the Chinese
national standards. In addition, the design documents of them shall be appraised before
type testing. In the case of lack of Chinese standards, the manufacturer should report the
applied standard and related technical document to the safety supervision administration
of AQSIQ for review and approval. Among them, the key items related to the safety
quality of gas cylinder, such as, design temperature, design pressure, bursting testing,
NDE, mechanical properties, must not lower than the requirements as specified in the
corresponding Chinese national standard.

3. The color marking of the imported cylinders shall be in accordance with the provisions
in Chinese mandatory national standard GB 7144.

1. Supervisory inspecting the items related to the safety performance of the product
during boiler manufacturing.
2. Supervisory examining the implementation of the quality system of the supervised
manufacturer

III. Items and methods of supervisory inspection


1. Drawing and data review
(1) Review the design data of the boiler products,
(2) Examine the standards and technology for boiler manufacture and inspection;
(3) Review the standards and technology for non-destructive examination (NDE);
(4) Review the approval formalities of design change (including material substitution).

less than 30 per cent of the whole and should not less than 10 films (including the x-ray
films at T-joint, doubtful area and at the repaired area).

6. Heat treatment
Check and confirm the correspondence between heat treatment curve and actual heat
treatment parameter and procedure.

7. Pressure test
Before pressure test, confirm that all required items have passed the supervisory
inspection and the supervised manufacturer has kept the testimony of the
whole-completed work. During pressure test, the supervisory inspector shall check the
test equipment, instruments, and preparation work for testing and verify the test results on
the site.

8. Safety appurtenance
The number, specifications, types and certificates of compliance of safety appurtenances
should meet the requirements.

9. Leak test
Check the leak test result to assure that it meets to the requirement of concerned code,
standards, and design drawing.

10. Delivery technical data


(1) Check the technical data to be delivered
(2) Check the nameplate if the contents on it are in accordance with the relevant
requirements and the supervisory inspection stamp has imposed.

11. Supervisory inspection data


Once the product has passed the supervisory inspection, the supervisory inspector should

Check the type approval testing results of the gas cylinders.

all kinds of data meet the requirements of corresponding codes and standards and
design documents.
3. Check the materials of the gas cylinder proper; verify the chemical composition per
heat and the re-testing results. The supervisory inspection agency may conduct the
re-testing if necessary. Confirming the re-test results for macrostructure of material if
the gas cylinders are made of billet. Verifying the situation and results of NDE per each
tube if the gas cylinders are made of seamless steel tubes.
4. Check the identification of the acceptable material before and the transportation
identification of the after cutting.
5. Check the welding procedure qualification and its record, Confirm the welding
procedures used in gas cylinder production are conforming to the relevant standards
and codes. Check the proof test reports for heat treatment parameter and procedure of
seamless gas cylinders.
6. On-site witness the hydrostatic pressure tests of each gas cylinder and confirm the test
pressure, dwelling time, test results and the cylinder’s serial numbers stamped on each
gas cylinders whether have been recorded.
7. The supervisory inspector shall select and mark the sampling gas cylinders with small
or medium volume and record the serial numbers of them. The inspection results of the
appearance and other items, that require to be inspected one by one according to the
product standards, should accord with the requirements as specified in the relevant
codes and standards.
8. Check if the material of product welding test plate for big gas cylinders (>150 liters)
are in conformity with the material of the gas cylinder proper. Before product welding

test plate is cut off from longitudinal weld, the supervisory inspector should stamp the

9. The supervisory inspector should choose and mark one sample cylinder per batch of
acetylene gas cylinders, and records its manufacturing number. When the sample

cylinder is sectioned, the inspector should check the gap between packing and wall of

the cylinder, packing appearance, surface holes and the preparation of packing sample
on the spot. During the test of compressive strength, bulk density and porosity etc. of
the sample packing, the supervisory inspector should conduct on-site spot check and
verify the test records.
10. On-site check if the process and test results of mechanical property testing conform
to the relevant requirements.
11. The supervisory inspector shall sample the gas cylinders for performing flattening

test according to the rules, check the preparation work before the test and witness the
test on the site. Check the span between plungers, flattened value and whether there is
crack in the flattened place under loading.

12. Check the section, preparation of cold bending test specimens for seamless gas
cylinders and the cold bending test method and test results of them.
13. Check the metallographic report. Check metallographic photo if necessary. As for gas
cylinders under heat re-treatment, check the test specimen and metallographic photo of

the sample cylinder. Check the section and preparation of the dissection specimen from
the bottom of gas cylinder, examine its macroscopic analysis result and measuring the
shape and size of the bottom structure of gas cylinder.
14. The supervisor should sample one test cylinder from every batch of products,
witness the hydraulic burst test on the site. Check test equipment, instruments and safe
protection measures before the test. Validate the test records and results.
15. Check the appearance, stamp mark, color and mark of the gas cylinder. The color
should be in conformity with standard color card.
16. Check the batch inspection report of delivery gas cylinders. Each gas cylinder should
attach with certificate of compliance with the stamp of supervisory inspector. The
supervisory inspector should put supervisory inspection stamp on each gas cylinder.
17. The supervisory inspector should review the following documents of the supervised
(1) Quality manual
(2) The list of appointment and dismissal of quality system personnel
(3) The list of personnel engaged in NDE (including items in certificate, level and term of
validity, etc.)
(4) The list of personnel engaged in gas cylinder quality inspection
(5) Manufacturing technological documents and inspection data as well as the list of
welding procedure qualification.
(6) Enterprise standards, design documents and fatigue test verification report for gas
cylinder.

IV. Quantity of gas cylinders to be supervisory inspected


1. Each beach gas cylinders must complete all supervisory inspection items listed in

I. Applicable scope
This outline is applied for all kinds of baric oxygen chamber for medical treatment with

1. Supervisory inspection the items concerning products' safety performance during the
process of manufacturing and installing baric oxygen chamber for medical treatment, i.e.,
including supervisory inspection during manufacturing and supervisory inspection during
installation.
2. Supervisory examining the implementation of the quality system of the baric oxygen
chamber manufacturer.

III. Items of supervision inspection


1. Design drawing and related data
(1). Check if the design drawings of the chamber and of the main system of the product
have reviewed and approved in accordance with the relevant stipulation and if the
drawings has borne a design approval stamp so as to confirm the validity of their
qualification.
(2). Confirm the effectiveness of the standards for manufacture, installation and for

(6) Check the qualification of the personnel engaged in manufacture (installation) and
inspection of the baric oxygen chamber, including welders and welding operators,
non-destructive examiners and electricians.

(7) Verify whether the volume of the chamber per person meets the requirements in
relevant standards.

2. Materials
(1) Check the material certificate of the chamber proper and main pressure parts
(including plate, pipe for supply gas and for supply and exhaust oxygen).
(2) Check material certificate of observation windows, illumination window and the

organic glass for illumination windows. The material must free of obvious scuffing and
mechanical damage or aging silvery vestiges.
(3) Check if the decoration materials in the chamber meet the related requirements of
GB12130 or related standards for the baric oxygen chamber.
(4) Check the certificates of compliance of valves, sealing components and cables.
(5) Check the material substitution formalities for main pressure parts and components
and for organic glass.

3. Manufacture, inspection and testing


(1) Check the applied welding procedure qualification of the baric oxygen chamber.
(2) Spot-check the identification steel stamp and the qualification of the welder and
welding operator performing the welding of the chamber.
(3) Check the surface quality of welded joint, especially attention to the appearance of
fillet joint between observation window and chamber, supply passage and chamber,
and the chamber door and head.
(4) Check the x-ray film map and the flaw detection report. Verify the NDE detection rate,
x-ray position and assessment result of flaw detection.

4. Equipment in the chamber


(1) Check if the supply passage, chamber door in the chamber, which has quick-actuating
door structures opening outwards, has installed with safety interlock devices.
(2) Check if the manual operational device in quick-actuating door structures opening
outwards, which has equipped with the electric or pneumatic gearing device, can open
the door within the specified time.

(3) Check the connections of electrical cables and if they are easy to examine and repair

when the electrical appliances are fitted in the chamber.

(4) Check if the oxygen-collecting opening for oxygen gauge is placed in the middle of

the chamber and its outlet is out of the ornamental plate.
(5) Check the mark, reliability and sensitivity of emergency exhaust device.

between oxygen exhaust piping and the chamber.
(3) Check the leakage rate of the chamber during the leak test.
(4) Check the control valves at the high-pressure side of the oxygen manifold, check the
anti-malfunction device and warning sign at the connection between manifold and
oxygen gas cylinder.
(5) Check the outfits of gas storage tanks, air purifier and oil and water separators.
(6) Check the certificates of compliance or quality documents of flow meters;
thermometers pressure gages and other instruments.

7. Heat treatment
Check the area or parts of the baric oxygen chamber, which require heat treatment.
Confirm the heat treatment parameter and procedure and its actual report.

8. Safety appurtenances
(1) Check the quantity, accuracy and scale of the pressure gauges in the oxygen chamber.
(2) Check the type, quantities and set pressure of the safety valves on the chamber proper
and on the auxiliary pressure vessels.

2. Except the items listed in this outline to be supervised and inspected during both

inspection during manufacture should not undergo repeated supervisory inspection
during installation.

3. The agency, which undertakes the manufacture supervisory inspection, should sign on
the items that pass the supervisory inspection for confirmation and issue the
Manufacture Supervisory Inspection Certificate. The agency, which undertakes the
installation supervisory inspection of oxygen chamber, should sign on the items that

have passed the supervisory inspection for confirmation and issue the Supervisory
Inspection Certificate and stamp the Supervisory Inspection Mark on the nameplate of
the oxygen chamber.
4. The supervisory inspector shall put “acceptable” on the “Supervisory Inspection
Result” column for those items which are conform with relevant codes and standards
and put the name and serial number of their testifying paper with his/her signature on
the “Working Testimony” column. The supervisory inspector should put the measuring
data or existing problems on the “ Supervisory Inspection Result” column and
describing the actual situation of non-conform with the relevant codes and standards in
detail in addition to the handled measures of supervised manufacture on the “Note”
column, if the items are not conform to the relevant codes and standards.

Appendix 15 Revised Regulation for Boiler and Pressure Vessel Manufacture


Licensing

Since the“ Supervision Administration Regulation for Manufacture of Boiler and
Pressure Vessel” and “Requirement for Boiler and Pressure Vessels Manufacture
Licensing” came into effect on Jan.1st, 2004, AQSIQ has made revisions and

interpretations to same of the provisions, which are summarized as following.

1. NDE person requirement (Article 11 and 19 of “Requirement for Boiler and Pressure

“RT Ⅲ and UT Ⅲ” person required by the manufacturer of boiler A level , pressure
vessels A1 and C level was adjusted from “Level Ⅲ” to “ holding the certificate of RT
and UTⅡ(or above level) not less than four years”

2. Requirement of heat treatment and rolling machine (Article 22 of “Requirement for
Boiler and Pressure Vessels Manufacture Licensing”)
2.1 Heat treatment equipment required by high-pressure vessels manufacturer of A1 level
in “Requirement for Boiler and Pressure Vessels Manufacture Licensing” has been
changed to below:
For the high-pressure vessels manufacturer of A1 level, heat treatment performed by
outsourcing through corresponding qualified sub-contractor is acceptable.
2.2 Rolling machine required in “Requirement for Boiler and Pressure Vessels
Manufacture Licensing” has been changed to follow:
Manufacturer of pressure vessels shall have the bending ability suitable for its main
products. Sub-contacting of steel plate bending is permitted if the machine can’t roll
some individual products. However, the final weld for longitudinal seam shall be
completed by the manufacturer of pressure vessel.
On the situation of above mentioned, the corresponding quality control procedure or
document for sub-contracting shall be submitted together with the application
documentation while manufacturer begin to apply for the manufacture license.

3. Lifting ability
Lifting activity may be sub-contracted if the crane can’t meet the requirement for
lifting.

4. Item 5 of Article 55 in “Requirement for Boiler and Pressure Vessels Manufacture


Licensing”

“For the steel plate used for the tank of transportable pressure vessels, the sampling

quantity of the impact test is two steel plate per batch and the test temperature should be

c
-20℃ or per instruction on the drawing. The requirements for impact test and acceptable

.
impact toughness value should conform to Table 3.

5. Shell of the pressure vessels

The shell is the main pressure part of pressure vessel, the manufacturer shall have the
capability of producing the shell of pressure vessel and normally the sub-contracting is
not permitted. However, for the forged or foundry pressure vessels without longitudinal
seam, shell fabricating performed by the subcontractor who has the adequate production
conditions is also acceptable. Under the outsourcing, the subcontractor’s production
conditions, quality assurance measures, etc. shall be evaluated in accordance with the
pressure vessel manufacturer’ own quality guaranty systems. The subcontractor shall be

responsible for the shell quality of the pressure vessel, such as geometric dimension,
chemical composition, mechanical properties, the results of impact test (if require) and
NDE, etc.

Accumulator assemblers who do not have accumulator shell production ability
cannot be granted the pressure vessel Manufacture License, however, they are allowed to
purchase and assemble accumulator shell produced by manufacturers who have obtained
the corresponding Manufacture License, and on the body of the accumulator the

accumulator shell manufacturer and its license number shall be clearly indicated.

6. Qualification and classification for small size pressure vessel


The term” small size pressure vessel” refer to the pressure vessel which diameter less
than 150 mm or the content less than 25 liters. The pressure vessels aforementioned is
subjected to “Regulations of Safety Supervision of Special Equipment”, but are not
applicable to the“ Supervision Regulation on Safety Technology for Pressure Vessel”, so
these vessels may be designed and fabricate by any manufacturer holding the license of
A1, A2, A3, D1, or D2 level. Simultaneity, small size pressure vessels are not classified
in accordance to “Supervision Regulation on Safety Technology for Pressure Vessel”.

7. Gas cylinder container


Manufacturer of gas cylinder container with whole assembling volume no less than
one cubic meter shall obtain manufacture license of C3 level (assembling for gas cylinder
container only).
The basic requirement for pressure vessel C3 (assembling for gas cylinder container
only) shall be as follow:
(1) In order to guaranty the efficient performance of quality systems, the quality
management systems shall be established; the quality control documents and regulation
shall be completed too.
(2) The workshop is required to meet the production of assembling the gas cylinder
container.
(3) The gas cylinder container shall pass the corresponding inspection by China

e
Classification Society (Briefed as the CCS).

s
(4) The facility for pressure test and gas tightness test is required to assemble the gas

c
cylinder container.

.
(5) The professional technician and operator is needed and not less than 3 person
Manufacture license of C3 (assembling for gas cylinder container only) is not

w
required for the license holder of B1 (Seamless gas cylinder). However, above-mentioned
w
w
containers of gas cylinder shall pass the corresponding inspection by CCS, and the
inspection report shall be submitted to the license-issue organ for registration.

8. Addition the boiler and pressure vessel production site.


8.1 The manufacturer of boiler and pressure vessel, who desire to add another production
site beyond the approved address under the same company name due to the increase of
production ability, shall deliver the application to the license-issue organ. Upon the

application accepted and approved, the applicant is required to invite the audit organ to
conduct the manufacturer survey to the additional production site. After the assessment
passed, the name, address, manufacturing level, products variety approved at the

additional production site will be merged into the license scope of general factory (head
office), and no independent license issued only to the additional production site.
8.2 The subsidiary company who has business license or possesses the legal person status
can apply for boiler and pressure vessel Manufacture License independently according to

C
its qualifications, the Supervision Administration Regulation for Manufacture of Boiler
and Pressure Vessel, the Requirements for Boiler and Pressure Vessel Manufacture
Licensing, and the Procedures for Manufacture Licensing of Boiler and Pressure Vessel.
If the subsidiary company does not apply for Manufacture License by itself, its mother
company can apply as indicated by 8.1.

9. Change of manufacture address


With the reference to the Article 20 of “Supervision Administration Regulation for
Manufacture of Boiler and Pressure Vessel”, refer to the change of production site, the
form of “Application form for the change of special equipment manufacture license”
shall be filled out and delivered to the license-issue organ. After the application accepted
and approved, the manufacturer shall invite the audit organ to perform the manufacturer
survey for this change confirmation and the revised license will be issued after the
assessment passed finally. Production activities of boiler and pressure vessels shall be
suspended while site moving. Any production at the same time of removal is not
permitted in principle.

10. Scope extension of license


The license holder of pressure vessel including D level, or pressure vessel
manufacture holding level B, A3, A4 or A5, shall deliver the application to the AQSIQ
for the adding of D level. Upon the application accepted and approved, audit organ who
has the assessment qualification applicable for manufacture licensing of D level shall be
invited to perform the manufacturer survey and the corresponding audit report shall be
submitted to AQSIQ for approval.

11. Expiry date of boiler and pressure vessel manufacture license

The manufacture license of boiler and pressure vessel is valid for a period of 4 years

since the date of issue. If license upgraded from level D to level A, B, or C, the license
expiration shall be re-calculated from the date of upgrading approved. Within the
