Taxpayer List (Sales Tax) of the FBR during working hours.

➢ The firms will adopt Single Stage Two-Envelope Procedure as provided in the Public Procurement Rules
2004, vide clause # 36(b).

➢ Firms will quote their rates without GST; GST will be paid as per policy of FBR Government of Pakistan.

➢ No tender will be received after due date & time. Incomplete, conditional and without deposit at call bids will not be
entertained.

hours after deposit of tender cost. The tenders are also available in institution website www.ath.gov.pk and
KPPRA Website. The tender documents can also be obtained on written request after deposit of tender cost.
In case of the request received through courier the bank draft in favor of undersigned inclusive of courier
charges be submitted within due date. The closing date / time is 19-01-2023 at 10:30 am and will receive at
11:00 am which will be opened on the same date at 11:30 (a.m).

Request for Proposal (RFP)


Sealed proposals, on single stage, two envelope basis (Technical and Financial Proposals) are
invited for the supply of different equipment for Department of Polymer and Process
Engineering. The details of the equipment are available in tender documents.

The tender documents can be obtained from the office of the undersigned from the day of
publishing of this advertisement. Interested suppliers can get the tender documents on non-
refundable payment of Rs.1,000/- as tender fee, to be deposited in HBL, UET Account
No.01287902801499.

The bidders are requested to attach call deposit of 2% of the estimated cost of each equipment
as earnest money. The details are as follow.

 No telephonic/telegraphic/faxed/email tenders are acceptable.

The assessments and procurement procedure of the tender shall be governed by PPRA rules

DRESSINGS ETC PHASE-II FOR THE FINANCIAL YEAR 2022-23.

Dear Sir/ Madam

District Health Authority, Dera Ghazi Khan invites sealed RFP


(Technical & Financial) for the supply of Drugs /Medicines, Medical Devices
Including Auto Disable Syringes and Surgical Dressings etc Phase-II for
the FY 2022-23 on free delivery to Consignee’s end basis. Prequalification of
Pharmaceutical Manufacturers/Sole Agents of foreign manufacturers with
the DGHS is primary pre-requisite. Detailed technical specifications along
with quantities of Drugs /Medicines, Medical Devices Including Auto
Disable Syringes and Surgical Dressings are given in the RFP Documents.
2. The prequalified firms must participate in the bidding
process for their prequalified items against the total quantity, otherwise, its
status of prequalification may be recommended for cancellation
immediately by the concerned quarter and notification will be issued
accordingly for those products for which the bid has not been submitted.
The bidder must bid for entire/total quantity. Bid for partial quantity will
straightway be rejected.
3. Prequalified Bidders can download the RFP Documents containing tender’s
item specifications, quantity, terms & conditions from the websites of PPRA
as well as Primary & Secondary Healthcare Department
(www.pshealth.punjab.gov.pk) for information only. Same can be obtained
from office of the CEO, DHA D.G.Khan until the closing date for the
submission of bids.
4. Bidding shall be conducted through Single Stage – Two
Envelopes bidding procedure of Punjab Procurement Rules, 2014. The
envelopes shall be marked as “FINANCIAL PROPOSAL” and TECHNICAL
PROPOSAL” in bold and legible letters. The outer envelope shall clearly be
marked with Tender Enquiry No.2 for which the proposal is
submitted. Financial Proposal of bids found technically non-responsive shall
be returned un- opened to the respective bidders.
5. The last date and time for online bid submission is 23-01-2023 up till
12:00 AM via department’s online portal and the signed computerized
print of the same online submitted application along with hard copy of bid
must reach the office of the Chief Executive Officer, District Health
Authority, D.G.Khan on 23-01-2023 up till 12:00 AM which shall be
opened on the same date at 01:30 PM. The data submitted online on the
department`s online portal and submitted hard copy must be same.
6. The firms shall pay a non-refundable RFP/bidding document Fee of
Rs. 500/- per item of RFP/bidding documents at accounts branch of CEO,
DHA D.G.Khan.
7. All bids should be submitted in Tape Binding and properly
sealed in envelopes. All documents should contain proper page marking,
attached in sequence as indicated for evaluation in the RFP Documents
and signatures of authorized person. Moreover, signing and stamping of
each page of bidding documents/form is mandatory.
8. In case the date of opening or last date of sale is declared as a
public holiday by the government or non-working day due to any reason, the
next official working day shall be deemed to be the date of submission and
opening of tenders accordingly. The time and venue shall remain the same.

1) The Procurement/Bidding Process shall be governed by
the Punjab Procurement Rules, 2014.

2) Item(s) shall be quoted in Technical & Financial Proposal


with both Brand Name(s) and generic name.

3) The bidder shall attach unhidden photocopy of 2% Bid Security


of estimated cost of each item as mentioned in RFP
Documents, in the form of Bank Draft/Bank Guarantee/Call
Deposit Receipt (CDR)/Pay order/SDR, with Technical
Proposal (hard copy) and Original with Financial
Proposal.
