AFFIDAVIT OF CHANGE OF VENUE

We, JONATHAN NAMONNE ASUNCION and MARIETA CARIO PUGONG,

both of legal age, Filipino citizens, and residents of Purok 6, Magsaysay, Diffun, Quirino, and
Bua, Kasibu, Nueva Viscaya, Philippines, respectively, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. We are the contracting parties for marriage solemnization and hereby request that the
same be conducted at our residence;

2. We intend to celebrate our marriage on April 29, 2023 (Saturday) at 8:00 o’clock in
the morning at Upper Centro, Bua, Kasibu, Nueva Viscaya;

3. We further intent to have our marriage be solemnized by Pastor John Ray L. Betito as
our solemnizing officer;

4. We are executing this Affidavit in compliance with regards to the change of venue for
marriage solemnization as required pursuant to Article 8 of the Family Code of the
Philippines;

5. Finally, we are executing this affidavit to attest to the truthfulness of the foregoing
facts and for all legal intents and purposes it may serve well.

IN WITNESS WHEREOF, we have hereunto set our hands this 3rd day of April 2023 at
