Activity (Midterm)

The nurse conducted a home visit to the family of Mr. Tomas Hernandez. The following were the
initial data that she was able to gather:
Mang Tomas 42 years old, has been coughing for 3 weeks now and has hemoptysis. He has a
medical history of PTB, underwent treatment, and was declared cured of PTB in 2014. His
wife, Aling Linda, recognizes the need for him to undergo sputum microscopy. She
verbalized “Ganyan din yung sintomas na ipinakita nya nung sinabi ng doctor na may TB
sya. Natatakot kami nab aka maulit yon. Balak naming ipatingin sya sa center sa susunod na
lingo”.
Tomasito, 4 years old, weighs 12.5 kgs, looks pale, and is noticeably underweight, as
manifested by the evident bony prominences. Aling Linda verbalizes “Naku, mahina lang
kumain ang batang yan. Palibhasa nauuubos ang oras sa paglalaro. Tsaka naisip ko, nasa lahi
naman talaga naming ang hindi tabain siguro payat din si Tomasito”.
The nurse observed Aling Linda gave the child a small pack of fish crackers for his morning
snack. Aling Linda is the housewife, while Mang Tomas earns a net income of 250 pesos per
day as tricycle driver.

Instruction: Please analyse the case scenario and accomplish the following tasks:

1. Enumerate health conditions/problems identified (at least 3):


- The mother’s inability to recognize that her son is suffering from malnutrition due to the
fact that the child’s appearance is observed by the bony prominences and noticeable
weight.
- The mother and father’s inability to provide a safe and well environment with one of the
father’s job as a factor for lack of enough money for health expenses.
- The mother’s failure to acknowledge the care her son needs such as his nutritional needs
with the mother providing unhealthy snacks for her child.

2. Determine priorities among health conditions/ problems based on 4 criteria, apply score.
