Team performance plan developed for Diana Community Services

Hi everyone I am presenting my team performance plan

Slide 2:

For the team of Support workers

For 6 months starts from 31st of May 2021 and ends on 31st of December 2021

Slide 3:

After identifying issues in team performance this 6-month plan is to resolve the issues of
communication issues and increased medication error in the Diana Community Services.

Slide 4:

The team improvement objectives of this plan would be

Address Customer Issues Resolution the whole team will be involved to achieve this
objective the start date would be 31st may and the treatment will be continued till the end of
this year.

Reduce the Medication Errors is another object which will be implemented to achieve by the
whole team and the implementation duration would be six months starting from the 31st May.

Slide 5:

Periodic review meeting every fortnightly will be used as an assessment method

There is always an option of Emergency meeting if the unpleasant serious incident happens.

Monthly meeting to update any change in the plan will keep the check on the effectiveness
of treatment plan.

Slide 6:

Employees’ Training will be conducted for customer communication on time.

Create and encourage employees to follow SOPs properly,

Slide 7:

 100% Customer Issue Resolution on Time


Data should show the improvement in the customer issue resolution speed.

 90% Decrease in Medication Error


No reported incidents of medication error should be recorded.

Slide 8:
 Customer Issue will not resolved on Time and the customers will not be satisfied from
the support workers and they will seek help from other community services.
 Medication Error will continue to increase and cause in serious results. It will damage
the Diana community services reputation.
The Diana community services will have to identify the employees who are responsible and
terminate them.
