Performance analysis report yield the following issues identified in Diana community services.

• Increase of staff complaints due to conflict between team members, bullying or


discrimination among employees

• Communication issues – which causes delays to resolve customer requests

• Increase of health and safety incidents

• Poor customer service – it decreases customer satisfaction

• Medication errors… providing wrong medications to the users

Out of these two most pressing issues are selected for the report those are:

• Communication issues and Medication errors

This report discusses the performance analysis results of the employees of


Diana Community Services in the last three months. This report includes
communication issues and increased medication error. The Diana
Community Services conducted regular monitoring and concluded
shortcomings to assess training.

The purpose of this performance report is to outline the progress of the


Diana Community Services and establish corrective measures. This report
focused on the employee’s performance on 3- months’ performance-based
analysis measured through communication and proper medication delivery.
The analysis emphasized data gathering through a 360- feedback.

If we see the 1st problem which is Communication Issues

The current state of the problem is there are significant Delays to Resolve Customer Requests on
Time on the other hand the ideal state would be when the employees of diana community
services resolve all the Customer Issues on Time.

Similarly, If we see the 2nd problem which is Increased Medication Error


The current state of the problem is there are Increased of Complaints from patients about
Medication Error, whereas the ideal state would be when the employees of diana community
services commit no medication Error.

Communication Issues

Caused by the lack of Employees training, the listening and finding the solution of customer is
very difficult without proficient training of the employees.

Also,

Increased Medication Error is the result of the various versions of SOPs for the medication
creates confusion and inconsistent medication delivery.

The option of Gap Remedy is to mitigate Communication Issues Company has to Invest in the
proper employee training. And to research and implement the required training is Human
Resource Managers responsibility.

The option of Gap Remedy is to reduce medication errors is to Implement the standardize SOPs
for delivery of medication
