    INSTRUCTIONS

    1. Accomplish the student profile at the bottom of this page. Type the required information in the
    corresponding spaces.

    2. Copy a digitized 2” x 2” I.D. picture of yourself (recent) and paste it inside the box.

    3. Change the file name to lastname.firstname.Course&Section. Do not change the file format of
    this document. It must remain as docx.
    Example: Cruz.JohnLoydCE1A.docx

    4. Upload the accomplished student profile form to the page where you downloaded it and click
    the “submit” button.

    5. Submit on or before Sep9, 2022, 05:00 PM, Friday.

    IMPORTANT!
    Failure to accomplish this task will restrict you from accessing the next week’s lessons. Hence, you will
    not be able to download the assignment worksheet and the module for that week.
