    VISA APPLICATION CHECKLIST KOREA

    Original / Photocopy
    1. Duly filled up & signed application form
    2. Duly filled up & signed letter of atty.
    3. One (1) passport-size colored picture
    4. Original Passport
    5. Photocopy of Passport Bio-page (frontpage)
    6. Photocopy of the previous travel stamped & visas of any countries for the last 5 years
    7. Photocopy of previous and current valid visas

    IMPORTANT:

    1. The applicant has confirmed that he/she has no other documents to submit.
    2. The applicant was advised that failure to submit all necessary documents may result in the application being
    refused, but he/she has chosen to proceed with the application.
