Specific PPE Safety Helmet, Safety Glass, Safety Boot, Safety Gloves and 360 degree high visibility clothing or MA uniform,
required: Full body harness.
Specific Tools 3 meter warehouse ladder (ladder with handrail and platform)
Required:

Steps Pictures

1. MA equipment inspector shall:


a. Accept the inspection request by filling section
2 of the MA-EHS-S-004.2 Initial and Periodic
Inspection Request form. (see step number
17 for request form)
b. Send filled form to requester to notify the date,
time and location of inspection area.

Not appilcabe
2. MA equipment inspector shall Contact MA Security patrol
dialing 013 350 7444 to open the IIP centralized mobile
equipment inspection area and follow the below:
a. Shall call from approved assigned telephone number.
b. Notify the inspector name.
c. Specify the time to open the inspection area gate.

3. MA equipment inspector shall enter inspection area from


inner gate of MA public parking area and follow below:
a. Always follow posted signs and marking.
b. Follow identified speed limits; inspection area speed
limit shall not exceed 15 km/h.
c. Shall always wear seat belt.
d. Use indicators while turning right or left.

4. MA equipment inspector shall park his vehicle in designated Not applicable


parking area and follow the below:
a. Mandatory reverse parking.
b. Parking prohibited areas are:
i. Curbs
ii. In front of fire hydrants
iii. In zone marked no parking
iv. In area not designated for parking
c. Apply handbrake when parked
d. Do not leave the vehicle running.
5. MA mobile equipment inspector shall conduct equipment See the below photos
inspection by using MA-EHS-S-004.1 Initial and Periodic
Inspection checklist (see step number 17 for inspection
checklist) at ground level.
a. Inspections shall be conducted during daytime at
least 50 lux. Nighttime inspections are prohibited.
b. Conduct pre task hazard review (toolbox talk) with
operator and employees available at inspection area.

c. Make sure the work area is clean and free of any slip
or trip, & collision hazards before commencing work
d. Do not accept any molten metal handling
equipment’s such as MPTV and MTV’s without
washed to ensure equipment is cooled off.
e. Do not accept any forklift, pay loader, bobcat or
similar load carrying equipment with a load on fork or
bucket while brought for inspection.
f. Wear 360-degree high visibility vest or MA uniform.
g. Wear appropriate PPE (helmet, safety glass,
safety boot and gloves as minimum)
h. Ensure equipment is parked in designated
inspection area.
i. Ensure equipment parking brakes are applied (for
movable equipment).
j. Ensure proper wheel chocks are kept for equipment’s
not having brakes.
k. Use pedestrian walkways marked on the
inspection area floor.
l. Do not open any caps which can splash hot liquids
such as radiator cap, oil reservoir caps etc. while
inspecting.
m. Do not use any hand tools for inspection purpose.
6. Inspection requiring elevated platform shall follow item number
5 requirements and below additional:
a. Only use approved type of platform provided in
inspection other means of gaining height are
prohibited.
b. Safely place the platform at desired location and
secured the wheels by locking.
c. Make sure steps are cleaned off any slippery
materials.
d. During rainy condition platform shall not be used.
e. Use handrail provided while ascending and decanting.
Maintain three point of contact.
f. Secure the platform by applying chain on
connecting point.
7. Inspection requiring to go underneath equipment shall follow Not applicable
item number 6 and below additional.
a. Do not enter underneath while equipment is in
running condition.
b. Shall wear face shield when going underneath.
c. Do not tough any wet surface of the equipment.
d. Do not touch any engine, oil filter or hot surface
area.
e. Jacking up the equipment is prohibited while
inspection except manufacturer provided out riggers.
8. Inspection required to climbing on structure or body of the Not applicable
equipment designed by manufacturer. Follow item number 7
and below additional
a. Read and follow manufacturer requirement.
b. Wear full body harness and attached lanyard to an
anchorage point specify my equipment manufacturer,
if the structure height is more than 1.2 meter.
9. Fill results (Pass or Fail or N/A) on MA-EHS-S-004.1 inspection
checklist.
a. Fill equipment information details (from date of
inspection until inspector signature). This information
is mandatory.

b. Fill all the checkpoints (tick) of inspection items based


on actual result.
c. If any items is not applicable, N/A field shall be ticked.
d. No inspection item field shall not be kept blank.
e. If any inspection check points are missing from the
form could be included under other category.
f. Any faults observed shall be recorded under
findings log sheet.
10. After completion of inspection MA equipment inspector shall fill Not applicable
the MA-EHS-S-004.2 form section number 3 and handover to
MA security patrol to allow patrol to open the gate to release
the equipment.
11. MA mobile equipment inspector shall handover the completed Not applicable
inspection form to Central H&S focal point at Industrial Security
Administration Building First Floor to obtain equipment sticker.
Focal points are:
a. Mr. Aliyar Pareeth +966 (13) 350 7125
b. Mr. Mohammed Moiz +966 (13) 350 7122
c. Mr. Shabeer Ali +966 (13) 350 7124
12. Central H&S focal point shall:
a. Scan the inspection report (passed and failed) and
enter details in mobile equipment inspection
database.
b. Fill all applicable fields in database.
c. Maintain an excel searchable report in MA Connect
under EHS operational control program mobile
equipment folder.
d. Handover the equipment sticker to MA mobile
equipment inspector (only passed equipment) and
register the sticker issuance in register.
e. Fill the sticker issuance register.

13. MA mobile equipment inspector shall stick the sticker in


convenient location of equipment.
14. If the equipment is failed MA mobile equipment inspector
shall fill the faults observed under findings log sheet.
15. MA mobile inspector shall notify the requester if equipment
failed the inspection and handover the findings log sheet to
requester.
16. MA mobile equipment inspector shall ensure that inspection-
completed equipment’s are left the inspection area.
