Now you must complete steps 5 & 6 before your temporary license expires in order to receive your permanent license.
Permanent License
• One original 8" x 8" fingerprint card is provided by the State of Alaska, but you may also use this standard fingerprint card.
• Take the instructions, the card/form and photo identification to local law enforcement or other authorized agency to have the
fingerprints done.
• After you have had your fingerprints taken, you will submit all the required documents to the Alaska Board of Nursing.
4. Complete the Employment Verification form (p. 7 of application) providing proof of at least 320 hours of employment in a nursing capacity
within the two years
• If you cannot document 320 hours of employment in the past two years, you must satisfy the continuing competency requirements
of the Board or complete a Board-approved refresher course.
• If you have not practiced nursing within the preceding five years, you must submit proof of completion of a Board-approved
refresher course.
5. Verification of licensure from the state or Canadian province where you received initial licensure AND the state or Canadian province
where you hold a current license
• If the state(s) participate in Nursys, you must complete the Nursys verification process, pay their fee and designate Alaska as the
receipt of the verification.
• If the state(s) does not participate in Nursys, you must contact their state Boards directly for further instructions. You will need to
send them a Verification of License form to complete and submit to the Alaska Board of Nursing on your behalf.
If you graduated from a pre-licensure nursing program that was not taught with English as the primary language, you must provide
verification of passing one of the following English proficiency examinations with the following minimum scores:
• International English Language Testing System (IELTS) academic examination – overall score of 6.5 with a minimum of 6.0 on all
modules
• Test of English as a Foreign Language, a paper-based test (TOEFL-PBT) – overall score of 560 with a score on the Test of Spoken
English (TSE) of 50
• Test of English as a Foreign Language, computer-based test (TOEFL-CBT) – overall score of 220 with a score on the Test of Spoken
English (TSE) of 50
• Test of English as a Foreign Language, Internet-based test (TOEFL-iBT) – overall score of 84 with a speaking score of 26
Renewal License
Renewal of your license occurs every 2 years
• Registered Nurse renewal schedule: November 30th of even-numbered years.
• Licenses expire regardless of when first issued, except new licenses issued within 90 days of the expiration date. These licenses will
be issued effective through the next biennium.
• If you receive your original license within 12 months of the expiration date, you pay the full license fee and then at your first renewal
you pay a prorated renewal fee (half of the current license fee).
• You must renew your license on or before the expiration date to continue to practice in Alaska.
• There is no grace period to practice on a lapsed license.
Continued competency requirement - must complete 2 of the following methods:
• 30 contact hours
• 60 hours of participation in uncompensated professional activities
• 320 hours of employment as an RN or LPN
*Registered Nurses who receive their original license on or after December 1 of odd-numbered years are not required to provide proof of
continuing competency for their first renewal.
*Licensees are subject to a random mandatory audit of continuing competency. If you are selected for the audit, you will be notified in
writing and given 30 days to submit proof of continuing competency claimed for the concluding licensing period.

Requirements
1. Have a passing score on the National Council Licensure Exam (NCLEX) or the State Board Test Pool Exam (SBTPE).
• Have graduated from a nursing program and obtained a degree within the past 5 years.
• Obtained an advanced nursing degree in the past 5 years (i.e. LPN to RN, RN to BSN, masters, or doctorate).
4. Hold a Diploma, Associate Degree or Baccalaureate Degree in Nursing from an approved program or LPN applicants hold a Diploma or
Certificate from an approved program.
6. If you were educated in a foreign country, including Canada and Puerto Rico, you must follow the Foreign Educated Requirements for
further requirements.
• In order to receive a temporary license, you must not have a criminal history, a history or disciplinary action by a regulatory agency, a
pending complaint before the Board, or affirmative answers to any criminal background or disciplinary question in the application.
• If you submit an endorsement application online you cannot obtain an emergency walk-through temporary license.
• You must indicate on your application that you want to receive a temporary license.
Fingerprints
• You are required to submit a full set of fingerprints, for the purpose of obtaining a State and Federal criminal record checks.
• You are only exempt from this requirement if you have submitted a fingerprint card to the Arizona State Board of Nursing within the
previous two years.
• A copy of a clearance card issued by DPS is not a substitute.
• Submit your fingerprint card as soon as possible in order to expedite the process.
• A permanent license will not be issued until fingerprint results are received by the Board.
• It can take 6-8 weeks for the Board of Nursing to receive the results from your fingerprints and background check.

Process
1. Fill out the license verification form provided by the state in which you are seeking licensure. If there is no form, write and sign a letter of
request on a blank piece of paper. Be sure either form you use indicates your CA RN license number, your current name, the name you used
when you became licensed in CA and your birth date.
2. Main both the verification form and a check or money order of $100.00 to:

other state or country (indicating the license number, status, issuance and expiration date of each).
• If you do not have an active registered nurse license in another state, your license has been expired two years or less, and you have
practiced as a registered nurse within the two year period preceding this application, submit verification from one state in which
your license expiration date was less than two years ago.
• If you do not have an active nurse license in another state, your license has been expired two years or more, and/or you have not
practiced as a registered nurse within the two year period preceding this application, you must demonstrate competency to
practice and may be required to complete an approved refresher course, complete the Competency to Practice forms.
• You will be asked during the online application to attest to and provide information that you are lawfully present in the United
States or otherwise eligible to work here.
• The results of your background check from CBI are only available to the Office of Licensing for a period of 60 days after your
application is received. Therefore, you should complete the application within one week of submitting your fingerprints to CBI.
• You must submit to a background check specifically for your nursing license. You cannot use a previous background check or a
background check from another state or jurisdiction.
• You will be asked a series of questions concerning your practice at the end of your online application.
9. Provide supporting documents if you answered “yes” to any of the screening questions (if applicable).
11. If you were educated outside the United States and its territories must submit the Credentials Evaluation Service Professional Report
from the Commission on Graduates of Foreign Nursing Schools (CGFNS) (if applicable).

• Complete the Authorization of Release of Information form (p. 10 of application) and complete your Criminal Background Check.

nursing school for completion.
• If you have not been employed for at least the past six months and you did not graduate from nursing school within the past two
years (24 months) but you were employed as the same type of nurse for which you are now applying within the past five years (60
months), send a Nursing Employer Reference form to your most recent nursing employer(s) where you worked for at least six
• After completing the form, the employer(s) (or nursing school) must return the form by mail directly to the Board office. Forms
received from you will be rejected.
• If the state participates in Nursys, you must complete the Nursys verification process, pay their fee and designate Delaware as the
receipt of the verification.
• If the state does not participate in Nursys, you must contact their state Board directly for further instructions. They will need to
submit an official Verification of License form (p. 14 of application) to the Delaware Board of Nursing on your behalf.

verification letter from CGFNS (if applicable).

You can satisfy this requirement by selecting one option from the following five options.
• Applications are valid for one year from the date of submission.
• When mailing your application please use a 9x12 envelope and do not fold or staple any of the documents.
• The Board of Nursing does not provide a specific form to use when requesting licensure verification from states that do not
participate in Nursys.
• If you already have a Single State Georgia License and you want to upgrade it to a Multistate License and you meet the
requirements, fill out this application.

• A license application must be filed within 30 days of the fingerprinting to ensure that the results are obtainable from the HCJDC. If
the results are not obtainable, you will be required to obtain new fingerprints.
• The results take 3-4 days to get to the Hawaii Board of Nursing but you can receive a temporary license while your background
check is processing.
• If you are in a location that does not have Field Print, you can ask for hard copy fingerprint cards via email to the HI Board of
Nursing. The HI Board of Nursing will then email you instructions and mail a packet.
8. Submit documentation requirements if you answered "yes" to any of the background questions (if applicable).
Renewal License
Renewal of your licensure occurs every 2 years
• All licenses, regardless of issuance date, expire on June 30 of each odd-numbered year and are subject to renewal.
Process
• Renewal applications are made available about 60 days prior to the license expiration date.
• Required contact hours: choose one option from the following 10 options
• You may be selected at random to complete a Complete Background Check if you were licensed in Hawaii before July 1, 2017.
• Online renewal of a nurse license is available to licensees who have not had any disciplinary action pending or taken or have not
had a conviction during the two years prior to the renewal date.
• While permitted you are only allowed to practice nursing only if employed by employer indicated on your Verification of
Employment form. Once your permit is issued, no other will be reissued in the care of another employer.
• The Board of Nursing will only issue a temporary license when they receive all the above steps for license by endorsement. A
temporary license is used while the Board of Nursing is processing your information.
• A temporary license is issued under certain circumstances for instance, if you were educated outside the US and they are waiting
for your credentials to be evaluated. Or if they are awaiting your licensure verification from a state that does not participate in
Nursys.
• Prior disciplinary action or conviction, which has not been expunged, must be reviewed by the board.

apply for a temporary license as long as you are able to complete step 5.

• If the state participates in Nursys, you must complete the Nursys verification process, pay their fee and designate Idaho as the
receipt of the verification.
• If the state does not participate in Nursys, you must contact their state Board directly for further instructions. They will need to
submit an official Verification of License form to the Idaho Board of Nursing on your behalf.
5. A satisfactory nursing employment references from the three-year period immediately preceding the application.
• The reference must complete this Employment Verification form and either fax, email or mail it directly to the Idaho Board of
Nursing.
• Then either upload, fax or mail it directly to the Idaho Board of Nursing.
• Within your electronic application here in the Nurse Portal, you’ll be asked if you want us to mail a fingerprint card to you. If you
say “yes,” the Board of Nursing will mail you a fingerprint card, return envelope, and instructions; generally within 3 business days
of your request.
• Once you have had your fingerprint impressions made by a law enforcement official, submit the card to the Idaho Board of Nursing
by mail in the envelope provided. Do not fold the fingerprint card.

receipt of the verification.
• If the state does not participate in Nursys, you must contact their state Board directly for further instructions. You will need to send
them a Verification of License form to complete and submit to the Missouri Board of Nursing on your behalf.
• Request that your nursing education program submits an official final transcript. The transcript must include your current name,
date of completion, degree or diploma awarded, a signature if the director/registrar and the school seal or stamp.
• If you were licensed by examination in another state or territory and you received your nursing education outside of the United
States, in Puerto Rico or in Canada, you need to have a credentials evaluation service, approved by the board, send our office a
course-by-course evaluation report or have the Commission on Graduates of Foreign Nursing Schools (CGFNS) send our office
your CGFNS certificate.
• A Temporary Permit cannot be issued until the course-by-course report or CGFNS certificate and evidence that you were originally
licensed in another state or territory by a recognized examination are received and evaluated.

• Have developed and maintained a portfolio that includes the licensee's current continuing competency goals and evidence/
verification of professional activities to meet those goals.

• If you currently hold a license in another state that is not in your current name, submit a copy of a legal document indicating the
change of name (such as a copy of a marriage license, divorce decree, or court order).
• You must provide fingerprint images to be used for the purpose of permitting a state and national criminal history records search
through the OSBI and FBI.
• If you have an Oklahoma mailing address, you will need to make an appointment online and then go to an IdentoGo center

Renewal License

• Graduate from an international education program (approved by the authorized accrediting body in the applicable country and
verified by an independent credential review agency). See Accepted Credentialing Agencies below.
• Successfully complete the NCLEX-PN or NCLEX –RN exam.
• Provide verification of nursing licensure status and/or authorization to practice, if applicable, from country of origin.
• Successfully pass an English proficiency exam that includes the components of reading, speaking, writing and listening (except
for applicants from countries where English is the native language and the nursing program was taught in English and used
English textbooks).
• Provide a copy of your social security card AND another form of lawful presence (driver's license, birth certificate, passport, or
other items listed on the application.)
• Graduated from a foreign pre-licensure nursing education program within 5 years from date of application.
• Accepted transcript credentialing agencies:
• CGFNS course-by-course evaluation (Commission for Graduates of Foreign Nursing Schools); or
• CGFNS certificate program (CES); or
• ERES (Education Records Evaluation Service); or
• IERF (International Education Research Foundation, Inc); or
• Josef Silny & Associates, Inc.
• If you are a foreign educated nurse and are currently licensed in the US, you will submit an Endorsement Application instead of
the Foreign Educated Application.
• Issuance of your permanent license is dependent upon receipt of your Complete Background Check results from DCI.
• If you have answered “yes” to any of the eligibility questions, your application may be referred to the Application Review Committee.
