Appendix - Instruction to Additional Risk Assessment

-A scale of Consequence ( C : from 0 to 5) is used to indicate potential severity. The consequences are those credible
scenarios that can develop from a particular hazard.

-The Probabilities ( P : A-B-C-D-E) estimated on the basis of historical evidence or experience that the identified
consequences have materialized within the Company or ship.

-For the purpose of assessing Risks ( R : see following colors scale in ref of Appendix A –Ch 12.9) to health this axis
can be modified to provide an exposure rating for a given hazardous agent.

Trivial risk: No additional action required

Moderate risk: Incorporate cost/ effective risk reduction measures

Intolerable risk: Immediate corrective actions
