 Division C and D licences: Your respective Zonal Office

This addendum must be completed by persons, companies or groups that are, or have been, the legal
owner/s of mineral rights in Tanzania.

 Proof of ownership for all mineral rights in the form of original licence certificates
 Certificate of recording of transfer for licences acquired through transfers
 Proof of rent payments since the last renewal or since licence grant, whichever is the latest.)
