A. PROJECT CONSIDERATION
The consultant was asked to provide assistance in compliance activities of Ever
Engraving Stationary, Inc. located at 168 P. Gomez Street, San Juan. The activities
are Preparation of Compliance in Certificate of Non-Coverage, Discharge Permit,
Permit to Operate, Hazardous Waste Generator ID, PCO Accreditation, and Self-
Monitoring Reports.

B. SCOPE OF SERVICES
1. Registration of Online Accounts (Company Registration System, Pollution
Control Officers Online, Online Permitting and Monitoring System, Hazardous
Waste Management System Online, EMB Online Services – Self-Monitoring
Reports).
2. Provide liaison services between EMB – NCR & Ever Engraving Stationary,
Inc.
3. Prepare all the required documents for the application/registration of
Discharge Permit, Hazardous Waste Generator ID, PCO Accreditation and
mentioned online accounts.
4. Prepare and submission of Self – Monitoring Report for Four (4) Quarters to
EMB – NCR.

C. LIMITATION
1. Fees from penalties from previous violations are not covered in this quotation.
2. The fees for training program of Pollution Control Officer (PCO) & Managing
Head.
3. The preparation & submission of CMR & SMR covers only limited periods and
will expire once the terms are satisfied.
4. The project proponent shall provide the consultant with relevant information,
data, documents and layouts plan.

TERMS AND CONDITIONS

1. A 50% Downpayment is required to proceed with the proposal.
2. Consultation services is included limited for this project.
3. Corresponding refund shall only be made if the consultant fails to provide the
corresponding service/s.
