Consent Form for Candidates

CR3 Group (“Company”) recognizes the importance of the protection your privacy and personal data
(“Data Subject”). We follow security procedures when collecting, using, and/ or disclosing your Personal
Data including your Sensitive Data (collectively referred to as “Data Processing”) to compliance with the
Personal Data Protection Act (“PDPA”) with the main objectives according to the Privacy Notices for
Candidates. In this regard, I have read and acknowledged the Company's privacy notice and I acknowledge
that I have the right to withdraw my consent on the collection, use, and disclosure of personal data for
the purposes stated in this letter by notifying the Company in writing of such withdrawal of consent.

The consent to act in accordance with pursuant to the purposes, the Company may process your Personal
Data in accordance with applicable law and a legal basis. The legal basis on which the Company performs
such processing are as follows:

1. To conduct background checks and reference with, if it is possible, to the extent with your consent.

The consent for collecting, using, and/ or disclosing of my Personal Data, I make a voluntary and informed
decision to consent to or refuse as detailed in this consent letter.

In case you do not consent, the Company may be restrictions on the performance of legal duties,
security and/or a contract between you and the Company will arrange for the allocation of appropriate
facilities, activities and benefits from the Company's inability to use your personal data.
The Company has put in place appropriate security measures for personal data. To prevent the loss,
access, use, alteration, correction or disclosure of personal data without authorization or against the
law which is consistent with the Company's information security policies and practices; Privacy Policy.
