I, Pat Uncangco, (age) years of age, (status), residing at _________________, hereby

submits to your Court this attestation that I personally witnessed my brother,
__________ executing his Last Will and Testament before (Name of the Secretary),
secretary of his lawyer on ______________ at _____________

In the said execution of the Will, my brother assigned his real property, a house and lot
in ____________ with _____ square meters located at __________________ to
Teresita Uncangco, his wife. He transferred all his rights, interests, claims and
demands that he has in and to the said real property. I am privy with the details since I
was with the brother during the time he executed his Will. I have personal knowledge
that said execution had happened. It was unfortunate that the document was lost in
transition caused by the secretary and failed to reach the hands of his lawyer.
However, in accordance to the the will and intention of my brother, I plead to the
Honorable Court to grant the assigned property to Mrs. Teresita Uncangco as the
rightful transferee.
