Discharge Plan

Medication Instructed the patient to take the following home medication as prescribed by the physician Instruct patient not to stop taking the medicines especially antibiotic without consulting the physician. Explain to the patient the purpose of each drug and how to take it with regards to the right dosage to prevent over or under dosage. Inform the patient of the possible adverse effects of the drug. Tell her to notify the physician if specific adverse effects occur. Call your healthcare provider if you have any of the warning signs of sickness: fever, sever pain, redness or swelling in the incision site, foul smelling vaginal discharge, increase bleeding, back ache or severe abdominal pain or cramping (relieved by medication.) Exercise Instructed patient to avoid strenuous activities. Promote Range of Motion as tolerated. Encourage her to have adequate rest and sleep to regain energy. Treatment Encourage patient to comply with all the treatment prescribed to her by the physician including diet, rest, exercise, pharmacologic treatment and follow-up check-ups.

Advice patient to monitor blood pressure. Teach the patient how to change wound dressings and perform wound care. Report increasing pain, swelling, or opening or gaping of wound edges. Hygiene Emphasize importance of hygiene and hand washing to prevent infection.

Out Patient follow - up Advice to visit or have a following check-up with her attending physician. Diet Encourage her to take foods that are rich in high in calorie, this will promote repair and wound healing. Low salt, low fat Significant Others Take advantage of help from others. Instruct significant others to assist patient in performing the necessary interventions and to offer help when asked, encourage them to support the client emotionally and psychologically.

PROGNOSIS Ideal The prognosis in preeeclampsia depends on how carefully a patient is monitored. Very careful, consistent monitoring allows quick decisions to be made, and improves the woman's prognosis. Still, the most common causes of death in pregnant women are related to high blood pressure. The outlook for full recovery from preeclampsia is very good. Most women begin to improve within one to two days after delivery, and blood pressure returns to the normal pre-pregnancy range within the next 6 to 12 weeks including proteinuria. Baby complications can be mostly avoided if the condition is controlled. Preeclampsia usually does not cause permanent damage or adversely affect the long-term health of the mother, but risk of preeclampsia is greater in subsequent pregnancies. Prenatal care can dramatically reduce the complications and deaths of preeclampsia, because women who are diagnosed while preeclampsia is mild can receive treatment without any delay.

General Prognosis A pregnant that is detected earlier with preeclampsia would have a good medical prognosis provided with the patients adherence to the treatment prescribed, medical advice and health teachings provided. But based on the actual percentage criteria presented here, the overall prognosis is poor because of the fact that the patient doesnt know her condition earlier until giving birth to her baby. Due to this case, she delivered pre-mature baby through cesarean section.
