For DepEd Salary Loan:

Requirements for Livelihood Loan ( DepEd) To be accomplished by the borrower
1.) Application Form Provided by Land Bank (3 Copies)***(to be notarized upon loan
approval only)(certified by principal/DIV Head)
2.) 3 months latest payslip photocopy of previous 2 months payslips & original copy
of recent payslip
3.) Certified true copy of two(2) valid ID’s with 3 specimen signature (DepEd ID is a
must).
4.) Certificate of Employment with no pending loan application from GSIS and other
PLIs (certified by principal/DIV Head)
5.) Appointment (certified by principal/DIV Head)
6.) Authority to Debit (provided by LBP)
7.) Certificate of Leave Credits & Net Take Home Pay (provided by LBP)(certified by
principal/DIV Head)
8.) SOA/OS balance from PLI certified by employee (for buy out/take loan)
9.) Contact No. and Photocopy of atleast 2 valid ID’S (DEPED ID and GOVt Issued
ID is a must) of the principal with 3 specimen signatures

For Livelihood Loan (LGU & NATL AGENCY):

1) Transmittal Sheet,ESL Profile (2 hard copies & 1 soft copy)
2) Photocopy of company/office ID (3 specimen signatures)
3) 3 months latest payslip photocopy of previous 2 months payslips & original copy
of recent payslip
4) Certificate of Employment signed by head of agency
5) Appointment (certified by head of agency)
